Full Job Description
Job Summary and Responsibilities

As our Director of Care Coordination for Community Hospital San Bernardino, you will be responsible for the overall direction and management of care coordination and social services including planning, organizing and directing all activities, staffing, performance improvement in the delivery of clinical services (such as LOS reduction) and reporting needs within the Medical Center and Dignity Health, as well as government and regulatory reporting.

 

Every day, you will work closely with all departments at the Medical Center and the post-acute service providers to streamline the patient transition through the health care system and into the community post discharge, and consult/collaborate with other managers, physicians, administration, and community based healthcare workers regarding care management issues identified through corporate or facility initiatives and current literature.

 

To be sucessful in this role, you must be competent and confident. Utilizing your expertise and excellent communication skills one-on-one and in front of groups, you will tackle tough conversations and, above all, be an educator at heart. Job Requirements

Required Education and Experience:

  • Bachelor's Degree in Nursing (BSN) or Masters in Social Work (MSW) or equivalent educationand experience required.
  • Minimum of 5 years of clinical care coordination experience (Utilization Management, Denial Management, Case Management, Care Coordination), or 5 years of progressively responsible healthcare management experience required.
  • Extensive operational experience, program planning, implementation, staff development, and needs assessment in healthcare environment required.
  • Comprehensive knowledge of care coordinationand discharge planning required.

Required Licensure:

  • Current California Registered Nurse (RN) license, or California Licensed Clinical Social Worker (LCSW) required.
  • National certification in either of the following: CCM (Certified Case Manager), ACM (Accredited Case Manager) required orwithin 2 years upon hire.
