Qualifications

• High School Diploma or GED required; BA preferred.
• Current Massachusetts LPN or RN license required.
• One year of experience working with individuals with DDS (Department of Developmental Services).
• Basic computer literacy; proficiency preferred.
• Valid Driver's License with a good driving record.
• Medication Administration Program (MAP) certification preferred.

Responsibilities

• Assess and coordinate healthcare services for individuals, ensuring timely follow-up.
• Assist and transport individuals to medical appointments following DDS standards.
• Document all nursing interventions and updates in Electronic Health Records (EHR).
• Maintain and review medication and treatment orders and monitor for side effects.
• Support implementation of Individual Service Plans (ISPs) related to healthcare.
• Train staff on relevant medication policies and procedures.
• Ensure accuracy and compliance of health records with DDS regulations.
