$76,689.00 - $103,521.00
The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
Responsibilities:
GENERAL PURPOSE:
Provides individualized assistance, support and comprehensive coordination of care for patients and their families/caregivers, to eliminate barriers to timely care, facilitate flow through system, facilitate interactions with care team, increase patient and provider satisfaction, maintain point of contact with providers and serve as first point of contact for patients and families.
FT position available, M-F, days. No weekends, Holidays or on-call.
The Nurse Navigator will play a crucial role in guiding patients through the entire continuum of care related to outpatient physical medicine and rehabilitation patients. The Navigator helps to remove barriers for patients and provides resources. Assists physicians with patient questions, prescription requests, and disability paperwork. We provide high quality expert medical care for complex and complicated disabling injuries and illnesses with our PM&R outpatient physicians. The multidisciplinary team includes physicians, nurses, therapists and clinical technicians. We offer rehabilitation through the lifespan- pediatrics through geriatrics and our patients and families have seamless access to our services through dedicated staff at our state-of-the-art facilities. Responsibilities include telephone management, patient triage, and coordination of care. Must possess strong assessment and communication skills, be proactive and independent in decision-making and have good organizational skills. 1-2 years of nursing experience preferred. Previous care coordination or nurse navigator experience preferred.
