With a commitment to patient- and family-centered care, the Ambulatory Electrophysiology (EP) RN provides technical and clinical services in the Electrophysiology ambulatory (outpatient) setting by overseeing clinical care of patients with cardiac devices. The Ambulatory EP RN performs cardiac implantable electronic device (CIED) interrogations, with arrhythmia interpretation of ECG, measured data, evaluation of arrhythmia burden, interpretation of diagnostics, and programming of cardiac device, reporting to an EP Provider. The Ambulatory EP RN will be overseeing both in person device management as well as remote device monitoring. Works collaboratively with the acute care EP team to ensure an appropriate discharge plan is developed and includes follow up with MD appointment, home care, and EP outpatient center when indicated.
• Integrates Evidence-Based Practice, clinical expertise, patients, their family/designated care partner(s) preferences and cultural beliefs and values for delivery of optimal healthcare
• Collaborate with physicians and assist in the coordination and follow through of the cardiac clinical plans documented.
• Utilizes advanced skill set in the daily 1:1 evaluation and interpretation of ICD & PPM management.
• Assess and interpret arrhythmias both in office and on home remote monitoring for troubleshooting and intervention.
• Manage patients’ diagnostics in manufacturer remote monitoring websites, analysis and management of transmission, entering data in the electronic medical record (EMR), maintaining the data integrity with the system, and having the judgment to direct to the EP Providers, when appropriate
• Triage walk in patients into the office.
• Collaborate with Clinicians to assist in the coordination and follow-through of the cardiac clinical plan of care.
• Triages Electrophysiology patient calls to Clinicians utilizing advanced clinical nursing skills to assess the degree of needed response.
• Assures the completion of all necessary forms including disability, homecare, hospice, medical clearance and Pre procedure device evaluation forms.
• Facilitates the direct admission of patients, when necessary, both planned and emergent.
• Acts as a liaison for all pharmaceutical and device reps, homecare and hospice agencies.
• Enroll patients in Cardiac Rehab programs as appropriate.
• Completes Genetic testing prior authorization and ordering.
• Completes medication prior authorizations
• Responds to online patient messages through EPIC.
• Schedule all Electrophysiology procedures for patients both in the office and at home.
• Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson.
• Cover anti-coagulation program on an as need basis.
• Maintains ACLS to handle patient emergencies both in the office and from home.
• Provides consultation, education and support to patients and patient caregivers related to care needs
• Oversees transitions of care from the inpatient to the outpatient setting with the participation of the multidisciplinary team.
• Maintains effective working relationship with medical team and all agency personnel providing care to the patient.
• Maintains effective collaborative communication with multidisciplinary team members in maintaining quality care for the patient.
• Documents all care management activities for each patient/client.
• Attend educational activities and departmental meetings as required.

Job Description
- Graduate of an accredited school of nursing required; BSN or completion of BSN within 3 years of hire strongly preferred.
- 2+ years’ experience as a Nurse, in primary care or ambulatory setting preferred IP cardiology or ambulatory clinic experience preferred. patients strongly preferred. Knowledge of Electrophysiology diagnostic and therapeutic procedures including Ablations, Cardioversion, EP Studies and implantable procedures preferred.
- Current RN licensure in state of employment; AHA BLS certification
