Responsibilities
- Provides skilled nursing care to individuals in the day program, including but not limited to assessment of acute and chronic concerns and medication and treatment administration.
- Provides in-services on health issues to day program personnel.
- Completes and maintains documentation on individual care services and keeps confidential medical information secure.
- Communicates to Nursing Dept., appropriate day program personnel including teachers and administration of any acute and emergent health issues that arise with individuals enrolled at the day program.
- Communicates with Clinical Charge Nurse any emergency issues immediately and all health issues by the end of their working shift.
- Contacts the guardians of individuals enrolled at the day program if any acute or emergent health issues arise.
- Demonstrates computer proficiency.
- Attends meetings for individuals as requested that occur in day programs, coordinating input with the professional disciplines at Woods.
- Collaborates with guardians, Medical Providers, and multidisciplinary professional staff to implement individualized care for those enrolled at the day program.
- Collaborates with Medication Trained Staff that are assigned to the day programs, orienting and educating them on specific medical treatments, changes in medications, new medications, etc.
- Maintains a safe and clean working environment, managing organization of office, medical records, and nursing supply inventory. Anticipates medical supplies needed and orders as necessary.
- Ensures operation of medical equipment by verifying emergency equipment availability, troubleshooting malfunctions, requesting repairs, and maintaining equipment inventories on a routine basis.
- Participates in any regulatory inspections with preparation and monitoring of health related documentation.
- Performs all other duties as assigned.
Qualifications
- Current licensure as RN/LPN in PA
- Two years' experience in the field of Developmental Disabilities preferred
- Current American Heart Association issued Basic Life Support or Advanced Cardiovascular Life Support certification.
- Current Driver's License
- Must be able to transfer a minimum of 50 pounds
