Sinai Hospital's inpatient Care Management team provides discharge planning, transitional care coordination, complex care management, and utilization review through a multidisciplinary team of registered nurses, LPNs, licensed social workers, and administrative staff. While many functions are dictated by regulatory requirements, the core clinical focus is always on ensuring a safe and effective transition to the next appropriate level of care. Your work directly supports safe discharge planning, reduces barriers to care, and improves patient outcomes.
Job Summary: This role will Coordinate Complex Discharge Planning through
- Managing cases including but not limited to difficult-to-place, high-cost, and complex patients that require targeted length-of-stay and safe transitions of care.
- Manage Caseloads of up to 15 patients using a structured escalation process to identify complex discharge barriers and assume responsibility for collaboration with an interdisciplinary team to ensuring a safe discharge plan.
- Monitor patients to identify opportunities for early intervention, reduce discharge delays, and promote cost-effective outcomes though tracking resolutions for future outcome

Requirements: - Master's degree in Social Work
- LMSW or LCSW-C
- Minimum of 3 years of inpatient medical acute discharge planning experience or 2-3 years of Complex Case management (Inpatient or Outpatient) is required.
Pay Range: $29.00 to $51.27 Based On Experience
