Sturgeon County is seeking a temporary full-time (to approximately January 28, 2028) Community Outreach Worker to provide support, advocacy, and coordination of adult and senior services to ensure residents are not isolated and are active members of the community. The incumbent must be available to work evenings and the occasional weekend.

NOTE: Interviews will be ongoing as candidates are identified.

KEY DUTIES AND RESPONSIBILITIES

Providing Direct Support to County Residents (75%)
  • Provides information, referral, support, advocacy, and coordination of services for adults experiencing life challenges.
  • Coordinates assistance to residents including negotiation, advocacy, navigation, and access to provincial and federal government benefits.
  • Provides support, information and assistance to families who are caring for their adult family members.
  • Provides outreach services to adults and their families in the community including home visits, as necessary.
  • Facilitates support groups to address identified community needs.
  • Organizes supportive resources to individuals/families experiencing financial hardship.
  • Completes applications for residents to access financial support from the Living Well Fund.

Liaising with Community Partners (15%)
  • Partners with community groups to develop programs and services that meet the needs of adults.
  • Attends regular community-based meetings representing adult populations.
  • Supports special events and awareness/theme days related to wellness.
  • Develops relationships with community service providers to facilitate effective referrals for community residents.

Other Related Duties (10%)
  • Works within assigned budget and assists with financial and outcome reporting as required.
  • Researches and understands emerging social trends and community issues that impact isolated populations.
  • Adheres to applicable responsibilities and accountabilities as outlined in the Sturgeon County Health and Safety Management System.
  • Ensures client files are kept up to date in the appropriate software program.
  • Performs Emergency Supports Services duties as required.

KNOWLEDGE, EDUCATION, QUALIFICATIONS, SKILLS, ABILITIES, AND EXPERIENCE

Required:
  • Post-secondary degree in Human Services, Social Work, or another related field.
  • Valid first aid certificate
  • Valid driver's class 5 license with access to a vehicle is required.
  • An acceptable driver's abstract, and defensive driving is required.
  • Applied Suicide Intervention Skills Training (ASIST) ASIST certification or Mental Health First Aid Certificate is required. (Must be completed required within the first three months of employment if not completed prior to hire).
  • A clear Criminal Record (Vulnerable Sector) and Child Intervention Record Check is required.
  • A minimum of three (3) years' experience working with vulnerable populations.
  • Strong interpersonal communication skills and the ability to collaborate on, including the skills. Key to this position is the ability to work well with both internal and external partners to ensure the effective delivery of services and programs for adults and provide advocacy as appropriate.
  • The ability to take initiative, be resourceful, work under pressure and meet deadlines.
  • This position involves significant work outside of the office, including visiting homes, agencies, and groups within the entire County region.
  • Proficiency with all Microsoft programs and other related software.
