THE POSITION
Are you motivated to use your nursing expertise to guide community health efforts and support vulnerable populations? The Community Health Nurse role with the Department of Health allows you to take part in disease prevention, education, and direct public health services. You will work closely with partners, monitor health trends, and help improve outcomes across the region. This position offers a meaningful opportunity to protect and promote the health of Pennsylvanians.
DESCRIPTION OF WORK
This position supports statewide public health goals by assessing needs, guiding initiatives, and providing direct nursing services. The role helps strengthen community programs, prevent disease, and promote healthier outcomes for residents. As a Community Health Nurse, you will perform the following duties:
  • Health Assessment: Review community data to identify public health needs and guide planning activities
  • Program Coordination: Link department resources with community partners to support local health initiatives
  • Disease Monitoring: Track communicable diseases, enter surveillance data, and support control measures
  • Health Education: Provide instruction on public health topics to support awareness and prevention efforts
  • Clinical Services: Deliver immunizations, testing, and related services according to established guidelines
  • Community Outreach: Offer case management, guidance, and support to individuals and targeted populations

Work Schedule and Additional Information:
  • Full-time employment, 37.5 hours per week
  • Work hours are 8:00 AM to 4:00 PM, Monday - Friday, with a 30-minute lunch.
  • Work infrequently requires statewide travel and irregular or flexible work hours, including overnights, weekends, and holidays, in accordance with policy.
  • Telework: You will not have the option to telework in this position.
  • Salary: In some cases, the starting salary may be non-negotiable.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY
QUALIFICATIONS
Special Requirements:
  • This position requires active authorization to practice as a Registered Nurse in Pennsylvania.
  • All employees possessing an active temporary practice permit must obtain licensure as a Registered Nurse within the one (1) year period defined by the Pennsylvania State Board of Nursing

  • This position falls under the provisions of the Child Protective Services Law.
