General Summary of Position

MedStar Health is looking for a Community Health Coordinator to join our team at our Washington Hospital Center!

The Community Health Coordinator will support planning, implementing, and coordinating community benefit initiatives under the direction of the Program Manager. The Community Health Coordinator will work as a community liaison and serve as a bridge between community partners and hospital programs and associates.

Primary Duties:

  • Assists in coordinating planning and developing community benefits activities that support the hospital community health assessment implementation strategies
  • Captures and documents all community benefit expenditures in the Community Benefit Inventory for Social Accountability (CBISA)
  • Provides project planning support for implementation of the tri-annual Community Health Needs Assessment survey
  • Support the planning and execution of programs tasks through coordination with physician liaisons, program management, and workgroups to support community programs and outreach opportunities
  • Maintains comprehensive data racking, including preparing and tracking correspondence, grant proposals, document and contracts


Qualifications:

  • Bachelor's degree in Public Health, Social Work, Human Services, Psychology, Sociology required
  • At least 3-4 years developing, implementing and tracking results of community outreach in a healthcare setting is required


This position has a hiring range of

USD $59,820.00 - USD $101,836.00 /Yr.
