Full Job Description
As the Community Engagement Program Manager, you will manage community engagement activities and partnerships that connect members to culturally responsive resources, social services, benefits, and community supports. Working with internal teams and external stakeholders, you will coordinate initiatives that support successful transitions to community-based settings, identify access barriers, incorporate member and stakeholder feedback into program improvements, and track progress toward contractual requirements and program goals.

The Illinois Tailored Care Management Program helps individuals with complex health and social needs transition from nursing facilities and Specialized Mental Health Rehabilitation Facilities (SMHRFs) to community-based living. Through person-centered care management and coordinated support services, the program empowers members to live more independently and thrive in their communities.

What You'll Do:

  • Manages community engagement initiatives that support successful transitions to community living
  • Coordinates resource networks, gathers stakeholder input, identifies access barriers and service gaps, and improves outreach, referrals, member experience, and community stability
  • Participates in community, client, and state forums and coordinates engagement plans, issue resolution, performance monitoring, and cross-functional activities
  • Builds and maintains relationships with community organizations, providers, agencies, advocates, members, families, caregivers, and other stakeholders
  • Coordinates culturally responsive outreach, listening sessions, focus groups, advisory activities, and community forums that elevate member voices and connect individuals to needed community supports
  • Monitors community engagement participation, referral outcomes, partner responsiveness, resource capacity, access barriers, and expenditures
  • Ensures activities meet contractual and reporting requirements and communicates results, emerging needs, and recommended actions
  • Coordinates with interdisciplinary teams supporting community outreach, member engagement, resource navigation, and transition services
  • Promotes member-centered, culturally responsive, and community-informed practices and performs other duties and special projects as assigned


Required Skills and Experience:

  • Bachelor's degree in healthcare, human services, community development, or a related field, or equivalent training and experience
  • Five years of relevant experience
  • Demonstrated experience coordinating community engagement, building stakeholder relationships, and managing programs or projects
  • Strong written, verbal, and facilitation skills
  • Ability to travel locally and overnight up to 50%


Preferred Skills and Experience:

  • Master's degree
  • Experience with Medicaid or other government-sponsored healthcare programs, community-based services and supports, behavioral health or social service systems, community transitions, and addressing health-related social needs
  • Bilingual English/Spanish skills
  • Relevant community engagement or program management certification
