As a
Care Manager Supervisor, you will be responsible for supervising program staff. You will provide technical guidance and training to staff related to the contract/program operations. Care management services include: person-centered care planning, transitions of care, residential supports, behavioral health management, case/disease/disability management and quality improvement activities. You will be responsible for monitoring compliance in accordance with client and contract deliverables as well as service/accreditation requirements as applicable.
The Illinois Tailored Care Management Program helps individuals with complex health and social needs transition from nursing facilities and Specialized Mental Health Rehabilitation Facilities (SMHRFs) to community-based living. Through person-centered care management and coordinated support services, the program empowers members to live more independently and thrive in their communities.
What You'll Do:- Supervises day to day operation of staff to ensure compliance with program protocols, state requirements and URAC accreditation standards as applicable
- Ensures the operations of the contract/program are completed in a timely and satisfactory manner
- Selects, develops, and manages employees to achieve individual, team, and organizational success
- Ensures employee accountability through routine observation, performance measurement, and performance feedback

- Performs audits or quality monitoring of staff to provide feedback on strengths and opportunities for improvement
- Develops training protocols for staff and patient education tools to be used for orientation and on-going education, which addresses operating protocols, URAC accreditation and/or state requirements as applicable and workflow processes
- Stays abreast of health care trends and changes
Required Skills and Experience:- Master's degree in a Human Service, Social Science or health field and has a minimum of two years' experience in case management, at least one year of which shall be related to the elderly and disabled populations, or bachelor's degree in a Human Service, Social Science, or health field with a minimum of five years' experience in Case Management, at least one year of which shall be related to the elderly and disabled populations
- Licensed in Illinois as an RN, OT, LCSW, LCPC, or LMFT
- 3-5 years clinical experience required
- Minimum of 5 years' experience in leadership role or has demonstrated leadership capabilities
- Experience in working with individuals with developmental disabilities, serious mental illness, and complex medical needs
- Position requires valid driver's license
Preferred Skills and Experience:- Care management experience preferred
- Bilingual in English and Spanish a plus
$93,000 - $115,000 a year
