Clinical Supervisor

A Brief Overview

The Clinical Supervisor is responsible for ensuring that patient care is coordinated and managed appropriately and that there is oversight of the multidisciplinary home care staff. Assists in the coordination of the clinical and leadership activities of the Agency. Collaborates with the Director of Home Care in developing, assessing, planning, implementing, directing, and evaluating patient care.

What you will do
  • Supervises Home Care Nurses, Physical Therapist, Occupational Therapists, Speech and Language Pathologists, and Registered Dieticians by reviewing and evaluating each case through a variety of means such as joint visits, case conferences, interdisciplinary conferences, handoff reports and record review (clinical & billing)
  • Assists clinicians in identifying skills need, addressing social/safety concerns of patients, setting priorities and developing appropriate plans of care
  • Instructs and guides clinicians to promote more effective performance and delivery of quality services
  • Attends case conferences meetings with home care staff to facilitate coordination of care and discussion of interdisciplinary group involvement and patients' progress towards goals
  • Available during operating hours to assist clinicians as appropriate and necessary as a clinical resource or conflict resolution
  • Participate in Administrative On Call Rotation as scheduled
  • Acts as a liaison between staff, patients, organization & Agency
  • Manages scheduling process to ensure compliance with utilization, supervisions and appropriateness of skills
  • Collaborates with the Intake Team to properly manage new referrals, review patient information related to referral and assign appropriately. Conferences with the Director regarding any questions about an individual's eligibility for services, while complying with regulatory requirements
  • Participates in record reviews according to QAPI guidelines and communicates findings to Quality Members
  • Identifies staff educational needs and functions as a clinical resource/role model for nursing personnel
  • Assist in the screening and interviewing process of new employees and makes recommendations for employment of individuals. Assist in the orientation of new employees
  • Assists Director and other leadership staff in the planning, implementation and evaluation of in-service
  • Assists Director and other leadership staff in the review and revision of policies
  • Provides supervision to direct reports on an ongoing basis to ensure compliance
  • Works collaboratively with the Director to assure documentation is complete, timely and consistent with care
  • Responsible for day to day management of all clinical staff including annual performance reviews as required and any performance management issues
  • Manages staff PTO requests, holiday assignments and daily calendar
  • Seeks opportunities to further own knowledge in areas of responsibility
  • Competent in organizational skills to facilitate the delivery of care
  • Demonstrates a proactive attitude by anticipating potential problem situations and appropriately intervenes
  • Provides for a positive environment for clinical staff
  • Demonstrates effective communication with other departments
  • Responds to identified concerns of patients, physicians/providers
  • Performs other related duties as assigned


Education Qualifications
  • Graduate of an accredited school of nursing. required and
  • Bachelor's Degree BSN preferred


Experience Qualifications
  • 1-3 years Relevant experience in Home Care, Hospice and/or Community Health Setting. required


Knowledge, Skills, and Abilities
  • Strong leadership and clinical skills.
  • Excellent communication, both verbal and written as well as strong decision making, organizational and problem solving skills are needed


Licenses and Certifications
  • RN - Registered Nurse - State Licensure and/or Compact State Licensure Current New Jersey Registered Nurse License upon hire required and
  • DL NUMBER - Driver License, Valid and in State upon hire required and
  • CPR - Cardiac Pulmonary Resuscitation upon hire required

Pay Range: $115,003.00 - $132,496.00
