OverviewThe Clinical Staff Pharmacist is responsible and accountable for the provision of safe, effective, and prompt medication therapy. Through various assignments within the department, they provide supportofcentralized and decentralized medication-use systems as well as deliveroptimalmedication therapy to patients with a broad range of disease states. Clinical Staff Pharmacist proficiently provides direct patient-centered care and integrated pharmacy operational services with physicians, nurses, and other hospital personnel.
Responsibilities
- Responsible forproperly assessingpatients' pharmaceutical needs and providing pharmaceutical care.
- Responsible for theaccurateand proper compounding and dispensing of prescribed medications and solutions.
- Responsible for building andmaintainingpartnerships with patients and other healthcare providers to help build a strong multidisciplinary healthcare team that works to improve patient outcomes.
- Responsible for properly counseling patients/patients' families, andappropriate healthcare professionalsregardingdrug therapy.
- Responsible for daily pharmacy workflow and pharmacy operationsin the absence of thepharmacist-in-charge or designee.
Activities:
- Supervises and performs the functions involved in theaccurateand efficient dispensing of prescriptions.
- Reviews, fills,and verifies medication orders for accuracy and completenessfor dispensing.
- Monitorspatient'sdrug therapy to detect/prevent drug interactions, contraindications, and other adverse reactions.
- Screen all new medication orders forappropriate dose, drug interactions, allergies, and contraindications prior to dispensing medication.
- Counsel patients/patients'families, andappropriate healthcare professionals on all new medication orders.
- Makeappropriate interventionsto improve patient carethat is basedon current guideline-directed therapy.
- Makesappropriate interventionsto improve patient care.
- Uses and is proficient in the operation of the pharmacy computer system.
- Compounds and dispenses medications and other pharmaceutical agents/products.
- Obtains pertinent patient medication/medical information.
- Provides current andaccuratedrug information to patients and health care professionals.
- Communicates effectively with patients, health care professionals, and coworkers.
- Serves on hospitaland systemcommittees as assigned.
- Completesand properlydocumentsmedication therapy management interventions.
- Ensurescompliance with company and legal policies, procedures, and regulations.
- Ensuresconfidentiality of information, documentation, and records.
- Performsother related duties as assigned.
Qualifications
- Bachelor of Science degree in PharmacyorPharm D
- Current state pharmacist licensure in good standing orlicensure eligibility in state of employmentwithin90 daysof hire
- Immunization certification(if Retail or Specialty)
Preferred 1-2 years ofpharmacy experience
Maintain the training and education needed to provide patient-centric pharmacy services.
Retail:Will obtain andmaintainthe training and education needed to providecomprehensivepharmacy services, including but not limited to naloxone,immunization certification,medication therapy management,state and federal laws and regulations.
