Are you a passionate RN with a strong clinical background and a heart for seniors? Join Trinity Health Senior Living Communities as a Clinical Resource Manager—a vital leader in our Skilled Nursing Department who helps shape high-quality, person-centered care.

Key Responsibilities:

  • Lead and coordinate resident assessments (MDS), ensuring compliance with federal and state regulations.

  • Champion quality care by developing individualized care plans and working closely with interdisciplinary teams.

  • Oversee Medicare and managed care referrals and ensure accurate documentation and care coordination.

  • Serve as a mentor, coach, and leader for clinical staff, promoting excellence in care delivery.

  • Act as Chairperson of the Interdisciplinary Care Plan Team and attend care conferences with residents and families.

  • Monitor clinical workflows, staffing levels, and resource utilization to meet resident needs efficiently and effectively.

What We’re Looking For:

  • Registered Nurse (RN) with a current, active license in this state.

Preferred Qualifications:

  • Bachelor of Science in Nursing (BSN).
