Job DescriptionThe Clinical Program Coordinator, RN provides clinical leadership and program coordination for community-based preventive health, immunization, screening, and outreach programs. This hands-on role works with participants across the lifespan and helps expand access to preventive care, health education, and connections to ongoing healthcare and community resources.

ResponsibilitiesPrimary Responsibilities
- Coordinate community health fairs, preventive screening programs, vaccination clinics, educational programs, and outreach events.
- Provide clinical leadership and support for community-based preventive health and immunization services serving participants across the lifespan.
- Contribute to pediatric and community immunization initiatives, including the Children's Immunization Program, as operational needs require.
- Administer age-appropriate immunizations in accordance with approved protocols and recommendations.
- Ensure appropriate vaccine storage, handling, screening, administration, documentation, and post-vaccination observation.
- Conduct health assessments and preventive screenings, including blood pressure, blood glucose, and cholesterol testing.
- Educate participants and caregivers about preventive care, immunizations, chronic-disease prevention, and healthy lifestyle practices.
- Review screening findings, identify participants requiring additional medical evaluation, and provide timely follow-up.
- Facilitate referrals and warm handoffs to primary care providers and community resources.
- Develop and evaluate educational programs for participants, caregivers, staff, and community partners.
- Coordinate staffing, supplies, clinical workflows, and protocols for community programs and events.
- Collect and review program data and contribute to quality- and performance-improvement activities.
- Develop collaborative relationships with healthcare providers, schools, public health agencies, and community organizations.
- Serve as a clinical resource for Community Health Improvement programs and staff.
- Maintain compliance with organizational policies, nursing standards, applicable regulations, and public health guidance.
QualificationsEducational Requirements:- Graduate of an accredited nursing program; BSN or higher degree.
License/Certification/Registration Requirements:- California State RN License required
Experience:
- Minimum of three (3) years of relevant registered nursing experience in community health, public health, ambulatory care, primary care, pediatrics, immunization services, preventive care, population health, acute care, or a related clinical setting. Experience coordinating clinical programs, services, projects, or community-based events preferred.
