Job Summary:

The Clinical Practice Coordinator (CPC) facilitates coordination between multidisciplinary teams caring for patients within their designated service-line areas to ensure high-quality outcomes on the assigned units. The CPC will collaborate with local and national experts to drive innovative care, lead organizational and system change, and optimize resources to improve patient outcomes. They will provide clinical expertise, staff education, and lead and/or assist with system-wide initiatives. The CPC demonstrates comprehensive quality nursing care and will assist the medicine service team to deliver best practice/high-quality care, meet regulatory standards, certification, and accreditation. They will maintain effective relationships with nursing leaders and interdisciplinary colleagues while demonstrating professionalism and leadership within the institution. The CPC exemplifies Upstate's mission, vision, and values while practicing in accordance with Upstate policies and procedures.

Minimum Qualifications:

NYS licensed/registered RN. Master's degree in nursing and three years of post-licensure acute care experience required. Excellent written/oral communication, organizational skills, and strong problem-solving abilities required. CPR-BLS certification required by time of appointment.

Preferred Qualifications:

National certification in a specialty field strongly preferred. PreviousInpatient Nursing experience is preferred.

Work Days:

Monday-Friday Days. Evenings and Weekends as needed.
