The Clinical Pharmacist Specialist for Solid Organ Transplant is responsible for the provision of pharmaceutical care to abdominal organ transplant (liver, kidney, and pancreas), lung transplant recipients, and living kidney donors. The Clinical Pharmacist Specialist for Solid Organ Transplant is a designated member of the transplant team and will be assigned primary responsibility for providing comprehensive pharmaceutical care to transplant recipients in a culturally competent manner. Works with patients and their families and members of the transplant team including physicians, nurses, clinical coordinators, social workers, financial coordinators and administrative personnel of the transplant program. Performs or oversees a team of other healthcare personnel and support staff. Education Bachelor's Degree Pharmacy or PharmD Required Other PGY2 Pharmacy Residency in Solid Organ Transplant or a relevant combination of education and experience Required Experience 5 years experience in pharmacy institutional practice in organ transplant or PGY2 Solid Organ Transplant residency Required Licenses PA Pharmacist License Required Cert Pharmacotherapy Spec Within 2 years Required or Board Cert Transplant Pharmacy Within 2 years Required
