Clinical Pharmacist- MICU

Job Summary and Qualifications

Position Responsibilities:

  • The pharmacist is responsible for providing pharmaceutical care activities to ensure safe and effective drug therapy through the provision of pharmacy services.
  • Demonstrates knowledge of human growth and development, assessment, range of treatment, and care of patients appropriate to the ages of patients served.
  • Provides patient care activities to ensure safe and effective drug therapy through the provision of clinical services.
  • Coordinates and oversees the delivery of pharmaceutical services.
  • Provides monitoring and intervention in therapeutics to provide cost effective care with optimum outcomes.
  • Responsible for the provision of comprehensive clinical pharmacy services for the intensive care unit(s) (ICU) and all associated areas including pharmacokinetic and therapeutic consultation.
  • In collaboration with Clinical Manager, prepare formulary documents for new ICU requests, protocols, policies, and order sets and make recommendations regarding place on formulary. Develop guidelines to ensure appropriate use of high-cost and/or specialized drugs used in the ICU.
  • Participate in the development of medication use management programs within this facility including clinical guidelines, critical pathways, disease management, and drug use programs.
  • Acts as a preceptor to residents and/or students; must maintain minimum requirements as outlined by ASHP, including any certifications, and shall be limited to practicing as a preceptor in training for a maximum of two years.
  • Participate in the professional development and competency of clinical staff. Functions as a role model and as an educational resource for pharmacy staff.
  • Serve as a resource to the ICU. Provide formal and informal education to medical staff and other healthcare providers when applicable.
  • Keeps informed of all local, state and federal laws covering the storage, handling and dispensing of drugs and for interpreting each medication order to determine that it meets all legal requirements.
  • Maintains awareness of contemporary trends in the profession through the professional literature and regular attendance at professional organization meetings, institutes and seminars.
  • Provides patient specific medication teaching for discharge medications when appropriate.
  • Assist in obtaining medication histories and documenting medication allergies when needed.
  • Proficient in the use of Microsoft Office applications, Meditech or other required operating systems, Order Scanning Systems, Automated Dispensing Units, Packaging Equipment, Physician Order Entry, and Clinical Integration to systems.
  • Maintain effective communication with Skyline directors and leadership teams.
  • Supervise and direct support personnel.
  • Collaborate with staff and clinical pharmacists to ensure all pharmacy consults and high alert Vigilanz activations are completed each day.
  • Work with quality and pharmacy leadership on safety process improvements and all core measures.
  • Participates in special projects as needed and performs other duties as assigned.

What qualifications you will need:

  • Licensed Tennessee Pharmacist.
  • Three (3) years hospital experience or PGY1 residency completion
  • Board Certification in pharmacotherapy (BCPS) or another appropriate Board or pharmacy specialties certification PREFERRED
  • PGY2 residency completion PREFERRED
  • Excellent communications skills and good computer skills
