Clinical Pharmacist for Providence Home Services Specialty Pharmacy in Tigard, Oregon.

Full-Time/Day Shift

The Clinical Pharmacist for Credena Health is responsible for coordination of medication related services as it pertains to specialty pharmacy. Responsibilities include promotion of evidence-based, safe, effective drug therapy, standardization of medication practices across the continuum, and coordination with drug procurement, inventory management and billing processes.

Required Qualifications:
  • Education to meet certification, license or registration requirement.
  • Upon hire: Oregon Pharmacist License.
  • Minimum 2 years' experience in retail, specialty, or ambulatory care pharmacy practice

The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors.
