DAY SHIFT3 12 Hour shifts/week; Days rotate weekly; Includes 1 weekend per monthAPPLICATION MUST BE FILLED OUT ENTIRELY TO BE CONSIDERED FOR INTERVIEW PROCESSScope of ResponsibilityIn this role, you will take the lead on core clinical pharmacy services, including:
- Antibiotic stewardship and therapeutic drug monitoring
- Medication consults and drug information support
- Renal dosing adjustments and clinical interventions
- Verification and dispensing of medication orders
- Sterile IV admixture preparation
- Patient and staff education on safe medication practices
You will report directly to the Director of Pharmacy and work closely with nurses, physicians and other clinical team members.
Education- Graduate of an accredited School of Pharmacy
- Registered Pharmacist in the State of Texas
Training and Experience- IV certification required
- Hospital or inpatient pharmacy experience preferred
- Minimum experience: hospital internship rotation
