Summary:
The Clinical Pharmacist Coordinator, Infectious Disease (ID) & Antimicrobial Stewardship (AMS) provides clinical leadership and programmatic support for the System ID/AMS Pharmacy Team while maintaining an active role in direct patient care practice. This position dedicates 80% of time to direct patient care responsibilities and 20% to administrative, program development, and quality improvement activities.
The Clinical Pharmacist Coordinator serves as an advanced practice pharmacist/clinical lead for the System ID/AMS Pharmacy Team. This role functions as a subject matter expert and key liaison among pharmacy leadership, infectious disease providers, microbiology, infection prevention, and other interdisciplinary stakeholders to advance antimicrobial use, patient outcomes, and quality initiatives across the health system. The Clinical Pharmacist Coordinator will work collaboratively with the Clinical Pharmacy Manager and other leaders to support the planning, development, implementation, and evaluation of system-wide ID and AMS pharmacy services and initiatives while ensuring compliance with accreditation and regulatory requirements.
Responsibilities:
- Provides comprehensive clinical pharmacy services for patients on the ID/AMS services. Ensures safe, appropriate, cost-effective medication therapies for patients according to established policies, procedures, and protocols.
- Provides education and information related to ID/AMS to patients, pharmacy staff, medical staff, and nursing staff.
- Serves as the lead clinical pharmacist for the System ID/AMS Pharmacy Team assisting with the coordination of team workflows, clinical priorities, and system service expansion.
- Serves as a system clinical liaison for infectious disease and antimicrobial stewardship between the pharmacy staff, medical staff, nursing, and other hospital departments.
- Actively participates in committees, councils, and task forces focused on ID/AMS including co-chairing the Antimicrobial Stewardship Subcommittee of the System Pharmacy and Therapeutics Committee.
- Completes medication use evaluations, formulary addition requests, and therapeutic class reviews as required.
- Provides oversight of clinical team scheduling and coverage models, ensuring adequate staffing levels and alignment with patient care needs.
- Participates in the evaluation of drug shortages and collaborates with interdisciplinary teams on the development and implementation of mitigation strategies.
- Assists in the development, implementation, and continuous improvement of ID/AMS policies, protocols, and clinical practice guidelines to ensure alignment with current evidence and regulatory standards.
- Contributes to the development and advancement of system-wide antimicrobial initiatives to improve program effectiveness and maintain compliance with TJC and CMS requirements.
- Supports regulatory readiness initiatives and compliance with accreditation and antimicrobial stewardship standards.
- Partners with clinical and operational leaders to develop and monitor system-wide metrics for antimicrobial utilization, stewardship, and quality outcomes.
- Assists the Clinical Pharmacy Manager with tasks, strategic planning, program development, cost-containment initiatives, and implementation of departmental initiatives.
- Partners with pharmacy and physician leadership in the planning, design, implementation, and ongoing evaluation of infectious diseases and antimicrobial stewardship programs and initiatives across the health system.
- Serves as a mentor, preceptor, and role model for pharmacy students, residents, and colleagues.
- Performs all duties of the clinical pharmacy specialist as assigned and maintains competence required for position.

Required Job Qualifications:
- PGY-1 pharmacy residency or equivalent experience (minimally 3 years)
- Eligible for and achievement of Board Certification within 2 years of hire
- License-Pharmacist-March 31 (even yr)
Preferred experience
- Previous experience as a team lead or a residency program leadership position.
- Previous experience leading an interdisciplinary committee or workgroup.
- Experience leading infectious disease/antimicrobial stewardship initiatives and evaluating outcomes/metrics.
- Proficiency/experience with project management, quality improvement, clinical research and peer-reviewed publications.
- Active involvement in local or national pharmacy professional organizations related to infectious disease/antimicrobial stewardship.
