How you'll make an impact in this role- Enhance Patient Outcomes: Pro-actively manage chronic diseases (e.g., diabetes, hypertension) to prevent complications and improve overall quality of life.
- Optimize Pharmacotherapy: Perform comprehensive medication management, interpret labs, and safely adjust dosages under collaborative practice agreements.
- Boost Medication Adherence: Identify financial or insurance barriers and provide targeted education to streamline therapy and ensure access.
- Support the Care Team: Collaborate directly with providers to streamline workflows and ease provider burnout.
What minimum requirements you'll needLicensure / Certification / Registration:
- Pharmacist credentialed from the Oklahoma State Board of Pharmacy required.
Education:
- Bachelor's or advanced degree in Pharmacy required.
What additional preferences we're seeking- Pharmacist licensure by the state of Kansas prior to or within 6 months of hire date preferred
- Board Certified Ambulatory Care Pharmacist credential preferred
- Completion of a PGY-2 Ambulatory Care Pharmacy Residency in lieu of Ambulatory Pharmacist experience
