Qualifications

• Active RN license in Arizona (BSN preferred)
• 3+ years nursing experience in home healthcare preferred
• 2+ years of management experience preferred
• 2+ years OASIS experience preferred
• CPR Certification required
• Active Arizona Finger Print Card
• Clinical proficiency, strong communication skills, and compassion
• Team player and self-starter
• Knowledge of State and Federal home care regulations.
• Computer proficiency.

Responsibilities

• Ensure ongoing interdisciplinary patient assessments
• Oversee home care clinicians to meet clinical practice standards
• Supervise paraprofessional and licensed professional team members
• Manage assigned patient population effectively
• Develop education for patients and families to achieve care goals
• Ensure compliance with laws and regulations in patient care and office activities
• Identify and implement quality improvement opportunities
• Document patient complaints and maintain infection logs
• Conduct OASIS and chart audits
• Participate in QAPI program as requested
• Educate home health clinicians on documentation standards
• Stay updated on current trends in home care delivery.
