Full Job Description
Position Details:

  • Main Office location: Stoneham, MA
  • Service Area: Stoneham Center Territory
  • Job Type: Full Time
  • Schedule: Monday - Friday
  • Salary Range: $100,000.00 - $115,000.00

Responsibilities:

  • Lead and support the Center's clinical team, providing day-to-day guidance, coaching, and problem-solving to help clinicians deliver exceptional patient care.
  • Oversee daily clinical operations, including new admissions, patient assignments, visit completion, staffing needs, and continuity of care.
  • Lead daily clinical meetings to keep the team connected, address patient needs, identify barriers, and coordinate priorities. (In person once per week and virtual the other four days)
  • Serve as a clinical resource for staff, assisting with complex patient situations, care planning, documentation, physician communication, and other clinical needs.
  • Monitor patient care and clinical documentation to promote quality, regulatory compliance, timely billing, and positive patient outcomes.
  • Manage and develop team members through regular feedback, joint visits, case reviews, competency assessments, performance evaluations, recognition, and ongoing education.
  • Partner with Recruiting and leadership to identify staffing needs and participate in interviewing, selecting, onboarding, and retaining clinical team members.
  • Monitor clinician schedules, productivity, and caseloads while balancing patient needs, staff capacity, and appropriate use of per diem resources.
  • Support quality and safety initiatives, including incident review, infection control, patient feedback, staff education, and performance improvement efforts.
  • Collaborate with Administrative, Marketing, and other clinical leaders to support referrals, census growth, efficient operations, and a positive patient and employee experience.
  • Maintain clinical competency and provide patient or joint visits when needed to support patients and team members.

Qualifications:

  • This position requires candidates to hold an active RN, PT, or OT license in the state of Massachusetts.
  • Maintain current BLS CPR certification.
  • Minimum of two (2) years of direct clinical experience; home health preferred.
  • Prior supervisory experience preferred.
  • Excellent written and verbal communication skills.
  • Proficient with computers, tablets, smartphones, EMR systems, email, and other electronic systems.
  • Ability to foster a positive and purposeful culture.
