Clinical Liaison

Tacoma, WA - Covering Alaska Gardens and Heartwood Extended Care

In this regional role, you'll serve as the key connection between hospitals, families, and our facilities - helping patients transition to post-acute care with confidence and clarity.

You'll play a critical role in coordinating seamless admissions, building trust with healthcare partners, and ensuring patients receive the care they need in a setting that supports healing and quality of life.

Key Responsibilities:
  • Develop and maintain relationships with hospitals, physicians, discharge planners, and case managers
  • Evaluate patient referrals and assist with clinical screenings and pre-admission assessments
  • Coordinate admissions and ensure timely, efficient transitions from acute to post-acute care
  • Serve as a clinical resource and liaison to families and referral sources
  • Collaborate with internal facility teams to align care plans and meet resident needs
  • Track referral activity and participate in census development initiatives

Qualifications:
