Job Description:

The In-Patient Medicine Program at THP consists of a total of more than 350 beds across the CVH & MH and is staffed by large interprofessional teams who support the care of complex medicine patients within the Mississauga-Halton LHIN. Our in-patient units offer first-rate care in an innovative environment supporting the physical, mental, emotional and spiritual needs of our patients. Reporting to a Clinical Manager, the ideal candidate will be focused on providing excellent patient centered care in a dynamic environment where he/she will be given the opportunity to deliver care with compassion, excellence and courage.

POSITION SUMMARY

• Provides leadership and mentorship to interdisciplinary team members such as nurses, allied health to ensure comprehensive and integrated care planning occurs for patient to achieve outcomes that are of high quality and safe
• Oversees the integration of interprofessional care for a designated group of patients (within a unit / microsystem) and adapts based on changing conditions and priorities
• Coordinates, manages, organizes and prioritizes day-to-day patient care activities, staffing and resource utilization for the in-patient unit cluster
• Serves as a direct care provider for complex situations, as appropriate
• Assesses, coordinates and evaluates the plan of care with patients, families and interdisciplinary team
• Monitors patient's progress against plan of care until discharge from designated patient care area
• Acts as the primary liaison for physicians, interdisciplinary professional team, families and community resources
• Leads daily interprofessional team huddles and Quality Board huddles with focus on outcomes related to quality and provision of safe care
• Supports a patient service delivery system that fosters professional autonomy and accountability and patient-centred care
• Monitors and addresses patient and family satisfaction with care, health and safety issues along with the physical environment for patient care to meet our standards for best practices
• Facilitates individual and interdisciplinary teamwork and effective work processes
• Assesses risk management initiatives and ensures patient and team safety
• Critically evaluates and anticipates risks to patient safety; review and follows up on patient incidents entered into the Report System RL6
• Promotes development of clinical practice improvements and implementing evidence-based best practice that improve outcomes
• Provides patient/family clinical education using available tools and resources
• Assumes responsibility for educating, mentoring, guiding and evaluating the health care professional team as appropriate
• Creates learning opportunities and uses appropriate teaching/learning strategies, materials and technology to facilitate the learning of the interprofessional team at the bedside
• May participates in hiring, orientation and performance evaluation of nursing staff and the interprofessional team
• Serves as a professional role model, mentor and change agent to develop and assist staff
• Leads/facilitates unit-based teams to achieve unit goals & objectives
• Awareness and a good understanding of the policies and procedures that govern patient care and THP Practice Standards and follows the CNO standards of care
• Participates in the Health System and hospital-wide initiatives that support the strategic direction of the organization
• Utilizes new technology and evaluates its impact on staff, patients, families and overall best practices

Key Qualifications:

• Current certification with the College of Nurses of Ontario is required
• Bachelor of Science in Nursing required
• Masters degree is an asset
• Current CPR required
• Must be proficient in Peritoneal Dialysis to support nephrology patients-minimum 5 years recent experience.
• Experience using UKG, EPIC, MEDITECH
• Minimum five years clinical experience with a demonstrated understanding of clinical, operational and functional information and processes related to the Medicine Program
• Expert General Surgical nursing skills, knowledge and experience is required
• Previous experience in a formal nursing leadership role such as charge nurse and preceptor
• Experience working with an interprofessional team
• Experience as a patient advocate is a definite asset

Job Responsibilities:

• Your leadership, facilitation, decision making, conflict resolution and clinical abilities will promote and encourage staff empowerment
• Plans, organizes and co-ordinates the day-to-day activities on 2B Medicine, leading interprofessional rounds, renal rounds, coordinating patient discharges.
• Provides clinical support to staff and clients
• Identifies gaps in service and makes recommendations to address issues
• Designs, implements and evaluates educational programs/strategies that are evidence based and support best practice in the management of patient care
• Acts as a link with the Clinical Educator(s) and support the implementation of new educational initiatives for staff
• Consults and facilitates with community agencies if required
• Participates in the recruitment, retention and evaluation of staff
• Monitors quality indicators including patient satisfaction
