Description

The Clinical Director of Home Health is responsible for ensuring effective delivery of evidence-based practice of clinical patient care and compliance with federal, state and The Joint Commission standards and regulations. The Clinical Director serves in the regulatory position of Director of Professional Clinical Services (DPCS) and the Alternate Administrator for the Home Health License.Core Competencies
  • Creates a culture of effective communication and promotes positive working relationships with internal and external customers.
  • Communicates policies, standards, institutional changes, safety information and clinical information integral to the position.
  • Utilizes professional, respectful, effective communication to delegate and direct unit activities and patient assignments.
  • Communicates significant unit issues utilizing chain of command
  • Ensures unit based patient satisfaction scores meet or exceed the targeted benchmark
  • Utilizes follow up phone calls and field visits to communicate with patients and families to enhance the patient experience.
  • Follows up on all patient and family service issues within 24 business hours
  • Operationalizes the Shared Decision Making Model of nursing practice and integrates point of care staff into decision making.
  • Provides frequent informal feedback on work performance and uses coaching techniques that promote employee growth in work performance.
  • Addresses personnel issues in order to promote a healthy work environment
  • Follows through with disciplinary actions and employee grievances.
  • Facilitates prompt conflict resolution among employees.
  • Conducts a minimum of ten staff meetings per year.
  • Evaluates the quality and effectiveness of nursing practice, nursing services and healthcare delivery for assigned area and the integration of evidence based clinical services within their assigned unit.
  • Reviews and follows up on incident reports and reports trends to staff.
  • Uses quality control findings in determining needed areas of staff education or coaching.
  • Analyzes clinical outcomes data such as National Patient Safety Goals, Core Measures, Process Measures and infection rate and makes recommendations and takes actions to improve structure, system or outcomes.
  • Participates in multidisciplinary meetings/rounds to discuss and resolve patient care issues as needed.
  • Ensures payroll is submitted bi-weekly in accordance with the deadlines set by Payroll Office
  • Manages efficiencies of unit activities to meet established productivity standards, ie admission within 48 hours of referral or physician ordered start of care date.
  • Facilitates and supports recruitment and retention activities.
  • Delegates responsibilities appropriate to licensure, education, and experience of staff.


Department Specific Competencies
  • Works toward integrating Home Health and Hospice into the health care continuum system.
  • Represents Home Health and Hospice in the community to other institutions, public bodies, government agencies, organizations, services, groups, and persons.
  • Participates in the Quality Assessment Performance Improvement (QAPI) meetings to identify areas of opportunity for improvement in clinical documentation.
  • Incorporates identified areas targeted for further study, uses conclusions from reviewing trended data and recommendations from QAPI committee when formulating improvement plan.
  • Develops and maintains administrative procedures to assure fiscal accountability and control, equitable personnel practices, efficient office management, and the attainment of management objectives.
  • Ensures clear, effective working lines of accountability and responsibility between and within the service staff and administrative staff.
  • Assumes accountability for quality and fiscal control of staffing.
  • Demonstrates leadership in patient safety activities.
