Department: Ambulatory Multi Organ Transplant
Reports to: Manager
Wage Range: $45.85 - $57.31 per hour
Hours: 37.5 Hours per Week
Shifts: Monday to Friday
Status: Permanent Full-Time
Closing Date: July 16, 2026

Duties

As a member of the Ambulatory Transplant team, the responsibilities of the CLINICAL COORDINATOR, include:
  • Utilizing and applying current specialized knowledge and expertise on recent advances in nursing practice to facilitate the effective management of clinical activities, the coordination of patient care across the continuum and continuity in patient care management
  • Performing patient care responsibilities
  • Performing teaching/educational responsibilities
  • Participating actively in Quality Improvement initiatives
  • Participating in academic and research activities/projects
  • Performing administrative responsibilities
  • Performing cross-functional and/or other duties consistent with the job classification, as assigned or requested.


Qualifications
  • Bachelors of Science Degree in Nursing
  • Completion of a recognized masters degree nursing program preferred
  • Certificate of competency (licensure) from the College of Nurses of Ontario
  • Completion of Basic Cardiac Life Support (B.C.L.S.) program, Level II Rescuer certification
  • Up to three (3) years of relevant experience including experience working in an advanced practice role
  • Experience in Transplant nursing preferred
  • Effective organizational, interpersonal and communication skills
  • Excellent decision-making and problem solving skills
  • Excellent time management skills
  • Excellent teaching skills with the ability to teach one-on-one and/or groups
  • Ability to function collaboratively within a multi-disciplinary team and/or independently
  • Ability to work in a self directed manner
  • Demonstrated awareness of and adherence to professional and ethical standards
  • Client service oriented, with the ability to effectively work with diversity and appreciate that people with different opinions, backgrounds and characteristics bring richness to the challenge or situation at hand
  • Computer literacy
  • Research experience preferred
