Location: Milwaukee, WI

Facility:St. Joseph's Hospital

Department: Pharmacy

Schedule: Full-Time | 36 hours| Monday-Friday 8a-430p

How you'll make an impact in this role

  • Clinical Excellence & Advisory: Assess, review, and optimize complex medication orders to proactively resolve drug-related issues, recommending formulary alternatives and serving as a subject matter expert for interprofessional care teams and clinical leadership.
  • Specialized Dispensing & Safety: Oversee the precise compounding, dosing, and dispensing of medications, ensuring rigorous safety standards across specialized patient populations.
  • Interdisciplinary Collaboration: Direct-consult with physicians, nurses, and care managers to streamline treatment plans and provide expert drug information to healthcare staff.
  • Patient Advocacy & Education: Empower patients through direct medication counseling, helping them safely navigate and understand their therapy regimens.
  • Operational Agility: Adapt to dynamic clinical needs by providing flexible coverage across pharmacy units when required.

What minimum requirements you'll need

Licensure/Registration/Licensure:

  • Pharmacist credentialed from the Wisconsin Pharmacy Examining Board obtained prior to hire date or job transfer date required.


Education:

  • Bachelor's or advanced degree in Pharmacy required.

What additional preferences we're seeking

  • Doctor of Pharmacy
  • Minimum: 2 years of ambulatory care/chronic disease management experience.
  • Preferred:3-5 years of ambulatory care/chronic disease management experience and BCACP.
