Position Description

We're looking for a Charge Nurse (LVN/LPT) to provide advanced clinical care and team leadership while ensuring patients and families receive the highest quality of service. In this pivotal role, you'll collaborate with multidisciplinary teams, oversee patient admissions and discharges, and guide your nursing team to deliver safe, efficient, and compassionate care.

What You'll Do
  • Collaborate with medical staff, nursing leadership, and the clinical team to deliver safe, effective, and patient-centered care.
  • Coordinate admissions, transfers, and discharges, ensuring smooth workflows across shifts.
  • Delegate and supervise nursing staff, balancing workloads according to skills and abilities.
  • Support individuals served and their families in decision-making and preparation for discharge through education and advocacy.
  • Lead and participate in quality assurance and performance improvement initiatives.
  • Ensure infection control standards are followed and provide guidance to staff on compliance.
  • Oversee shift hand-offs, ensuring accurate communication and continuity of care.
  • Address documentation or care issues in collaboration with the Director of Nursing (DON).

What We're Looking For

Education & Training
  • LVN or Psychiatric Technician (LPT) degree from an accredited nursing school (required)

Experience
  • 2+ years as an LVN/LPT in long-term care, med/surg, or inpatient psychiatry (required)
