Full Job Description
Job Summary:
The Change Management Lead is a senior role accountable for leading change across SE Health's priority transformation initiatives, supporting the implementation of major digital, operational, and organizational changes.

This role sits at the intersection of program delivery and business implementation. It works with senior leaders, program teams, and operational stakeholders to translate transformation objectives into practical actions that support readiness, adoption, and sustained outcomes. The role operates in a complex, multi-stream environment where priorities evolve, initiatives overlap, and levels of readiness vary across the organization. Success requires the ability to take a whole systems view of business changes, underpinned by situational awareness and strong judgment on where to focus effort, how to sequence support, and how to help leaders and teams move through change effectively.

The Change Management Lead is expected to bring structure without overengineering, maintain focus on the areas of greatest implementation risk, and help ensure change is embedded into delivery rather than treated as a parallel activity. Over time, the role will also contribute to strengthening SE Health's broader approach to change through practical standards, repeatable methods, and stronger internal capability.

Key Responsibilities:

Transformation Change Leadership
  • Lead the change approach across major transformation initiatives involving digital, process, service, and operating model change
  • Translate transformation objectives into clear business, leader, and workforce implications for impacted groups
  • Integrate change considerations into planning, sequencing, implementation, and stabilization activities across initiatives
  • Make trade-offs on where to focus change effort, based on risk, impact, interdependencies, and organizational capacity
  • Help maintain alignment across initiatives where timing, stakeholder groups, or implementation impacts overlap

Adoption, Readiness, and Implementation Support
  • Establish practical approaches to stakeholder readiness, leadership preparedness, workforce impact assessment, and implementation support
  • Define and monitor indicators of adoption, readiness, and early stabilization, using them to adjust change approaches as delivery progresses
  • Partner with program and business leaders to connect change activity to intended outcomes, including uptake of new processes, effective use of new tools, leader follow-through, and transition into operational ownership
  • Identify risks to implementation early, including gaps in alignment, readiness, role clarity, or reinforcement, and support mitigation planning
  • Support the transition of ownership from program delivery into business operations

Leadership, Stakeholder, and Governance Support
  • Work closely with executive sponsors, senior leaders, and program leads to clarify the leadership actions needed to support successful implementation
  • Provide practical guidance on stakeholder alignment, decision-making, communications, engagement, and reinforcement
  • Support leaders in navigating resistance, ambiguity, and change fatigue across impacted areas
  • Contribute to governance discussions by bringing a clear view of organizational readiness, adoption risk, and change impacts
  • Help leaders balance implementation ambition with operational reality

Management and Leadership
  • Set direction and expectations for change managers and supporting resources across priority initiatives
  • Align work across resources to avoid duplication, close gaps, and maintain focus on critical areas
  • Determine where dedicated change support is required versus where accountability should sit with business or delivery leaders
  • Contribute to the development of practical standards, tools, and ways of working that can be used consistently across initiatives
  • Identify opportunities to strengthen internal change capability over time through coaching, role modelling, and repeatable approaches

Qualifications & Experience

Required
  • 10+ years of experience leading change in large, complex transformation environments
  • Demonstrated experience supporting enterprise initiatives involving digital, operational, service, or operating model change
  • Experience supporting multiple concurrent enterprise initiatives with overlapping stakeholder groups
  • Experience contributing to the development of change standards, approaches, or internal capability across teams
  • Strong track record of working with senior leaders in matrixed organizations and influencing across functions
  • Experience translating broad transformation goals into practical implementation approaches for leaders and teams
  • Strong judgment, prioritization ability, and comfort operating in ambiguity
  • Experience using adoption, readiness, or implementation indicators to guide decisions and adjust approach
  • Strong facilitation, stakeholder management, and communication skills.

Preferred
  • Formal training, education or Prosci certification in change management, organizational development, or a related discipline
  • Experience in healthcare, home and community care, or similarly complex service environments
