Caseworkers function as members of an interdisciplinary team to provide support to clients of the Mental Health Program. The primary purpose of the position is to assess clients, plan appropriate interventions and referrals, provide support and evaluate outcomes. Following a recovery model, the caseworker helps facilitate the optimal functioning of the client with a mental health illness.

REQUIREMENTS:
  • Minimum of a three (3) year university degree in a related field; social work, psychology, sociology
  • Minimum of two (2) years' experience in social services or healthcare
  • Minimum two (2) years' recent experience working with people with severe mental illness
  • Minimum of three (3) years' experience preferred
  • Provide full scale crisis intervention including: Emergency Departments, Police and Ambulance to effectively treat individuals in crisis
  • In collaboration with the client and his/her family, friends or other support network, develop a short-term plan to meet immediate needs
  • Facilitates short term emergency housing needs
  • Facilitates referrals to other service providers
  • Valid G Driver's Licence (or equivalent)
  • Has an available, safe vehicle, and clean driver's license record three (3) years prior to date of hire
  • Carries appropriate automobile liability insurance of no less than $2,000,000
  • Proven ability to effectively provide community-based service to people with symptoms of severe mental illness in Renfrew County
  • Proven ability to work in an interdisciplinary team with all members of the Health Care Team
  • Excellent interpersonal and conflict resolution skills
  • Excellent verbal and written communication skills
  • Knowledge of Renfrew County and community resources
  • Excellent organizational skills and ability to set priorities
  • Ability to work collaboratively as a team member
  • Ability to coach, mentor and influence
  • Proficiency in the use of computer applications and software programs
  • Must have demonstrated ability to meet the attendance standards of the Hospital
