Case Manager - Full Time - Thornton, Colorado

Denver Regional Rehabilitation Hospital, serving the greater Denver, Colorado area, provides specialized inpatient rehabilitative services in a warm and patient focused setting. We offer intensive therapy programs for patients recovering from strokes, neurological injuries, spinal cord and orthopedic trauma, and other debilitating conditions. Responsibilities

Works with physicians and multidisciplinary team members to develop a plan of care for assigned patients. Ensures patient is progressing towards desired outcomes by monitoring care through assessments and/or patient records. Identifies and resolves barriers that hinder effective patient care. Actively involved in discharge planning process. Integrates the hospital's mission and "Guiding Principles" into daily practice.
