We are seeking a Case Manager (LCSWs, MSWs, & RNs) to manage complex discharges for our partner hospital in the Silver Spring, MD area.
This role is ideal for a motivated, outcomes-driven professional who thrives in fast-paced environments and wants the flexibility, creativity, and autonomy to improve how patient discharges are handled.
This partner hospital serves a high-acuity, high-complexity population, including a significant share of uninsured, un- or under-housed, and behavioral-health patients-work that frequently involves shelter placement, guardianship, and patient identification.
This position allows direct access and communication with leadership and additional supportive resources.
What You'll Do- Facilitate safe and successful discharges for complex patients by navigating barriers and driving innovative solutions across hospital teams and post-acute care settings within SilverStay's preferred provider network, including SNFs, LTACHs, acute rehab, home health, and community resources. Caseload is an average of 10-12 per week, which may be a combination of complex cases and other cases to assist the hospital team with discharge efforts.
- Participate in SilverStay and hospital complex case meetings and engage with patients and families to ensure seamless communication across all stakeholders.
- Present reports to hospital leadership on progress, scalability efforts, and results, proposing and executing adjustments as needed to impact population health outcomes including LOS and throughput.
- Same-day handling of time-sensitive tasks (HINN/IMM letters, family conversations, payer calls) and daily documentation and charting so payers and leadership are able to act timely.
- Reprioritize in real time when leadership redirects focus to an urgent case-acknowledge, shift, and commit to a same-day timeline-without dropping the work already in motion.
- Build and refine scalable workflows and processes that increase efficiency and outcomes.
- Partner with SilverStay leadership and hospital teams to track key metrics (LOS, readmission rates, discharge success) and continuously improve results.
- Respond quickly, professionally, and reliably with hospital teams and others related to patient discharge planning.
- Escalate issues professionally and with urgency with hospital leadership and SilverStay leadership. Bring concerns and frustrations directly to hospital and SilverStay leadership-not laterally to floor staff or providers; engage proactively and constructively in defining case direction and prioritization.
- Flexibly adjust to changing hospital needs in real-time with thoughtfulness, a population health mentality and with grace for the customer.
- Leverage technology and data-driven insights to transform how complex cases are managed.
- Seamlessly work across hospital-related technology (EMR, discharge software), SilverStay technology (Slack, google suite, Claude) and participate in the development of Navigator AI (SilverStay's proprietary case management software).
Who You Are- Licensed RN, LMSW, or LCSW
- Experienced in adult, hospital, inpatient case management, discharge planning, or transitional care.
- Skilled at navigating post-acute settings (SNF, LTACH, acute rehab, ALF, group home)).
- A patient and family-centered communicator with empathy, professionalism, and persistence. Skilled at persuasion-able to bring families and providers along, including when the discharge plan changes late.
- Ambitious, entrepreneurial driven, innovative, dedicated and diligent work ethic.
- Positive and composed under pressure-solutions-oriented rather than complaint-first when priorities shift or a case gets difficult, and brings challenges to leadership constructively.
- Thrives in high-responsibility, high-ambiguity environments where adaptability and creativity are key.
- Flexible and adaptable in changing environments and with diverse team members and patients.
- A tolerance for ambiguity and process flux. This role is outcome-driven and workflows can shift week to week. Must be comfortable with change, while consistent with hours and on-site presence.
- Tech-savvy and excited about the role of innovation in healthcare.
- BLS Certified and able to successfully pass a background check and drug testing.
Compensation & Benefits- Competitive Salary: $100,000 - $130,000; commensurate with experience and potential impact.
