Full Job Description
As the Care Manager Trainer, you will lead the development, implementation, and continuous improvement of the tailored care management training program. You will prepare team members to deliver consistent, person-centered care through onboarding, role-specific orientation, core competencies, contract and regulatory training, continuing education, and competency validation. Working with program leaders, you will align curriculum with care management tools, workflows, documentation standards, and workforce performance needs.

The Illinois Tailored Care Management Program helps individuals with complex health and social needs transition from nursing facilities and Specialized Mental Health Rehabilitation Facilities (SMHRFs) to community-based living. Through person-centered care management and coordinated support services, the program empowers members to live more independently and thrive in their communities.

What You'll Do:

  • Develops and maintains the training strategy, curricula, educational resources, and training calendar for onboarding, role-specific orientation, continuing education, and contract and regulatory requirements; monitors completion within required timeframes
  • Leads staff training, onboarding, and competency validation
  • Provides ongoing education, coaching, and remediation to support workforce readiness and person-centered care
  • Partners with program and Quality Improvement leaders to align training with operational priorities, address performance and audit findings, and evaluate training effectiveness
  • Supports implementation of new workflows, policies, tools, and contract requirements
  • Maintains training and competency records to support documentation standards, audit readiness, and compliance
  • Stays current on care management practices, healthcare regulations, contract requirements, and learning technologies to strengthen training programs
  • Performs miscellaneous duties and special projects as assigned


Required Skills and Experience:

  • Bachelor's degree in nursing, healthcare, education, public health, social work, or a related field, or an equivalent combination of education and experience
  • Current Illinois RN license or other applicable healthcare provider licensure
  • Five years of healthcare, clinical operations, care management, workforce education, or related experience, including person-centered care and care coordination
  • Experience developing, delivering, and evaluating curricula, competency assessments, educational materials, and performance-improvement training
  • Demonstrated leadership, facilitation, coaching, communication, cross-functional collaboration, and change implementation skills
  • Knowledge of healthcare documentation standards, regulatory and contractual requirements, quality improvement, and corrective action processes
  • Valid driver's license


Preferred Skills and Experience:

  • Master's degree in a related field
  • Experience with Medicaid, managed care, behavioral health, social determinants of health, community-based services, care transitions, complex populations, or trauma-informed and culturally responsive approaches
  • Experience with adult learning, instructional design, virtual education, learning management systems, or competency-based education
  • Bilingual in English and Spanish
