  • Ensure compliance with regulatory requirements throughout the care process.

Care Manager RN - Care Management at Providence St. Vincent Medical Center. This is a Full-Time (0.9 FTE), Day Shift position.

RN Care Managers, in the acute care setting, provide a variety of services to appropriately help progress patients through the continuum of care. Key services include screening for post-acute needs; transition planning; care coordination; education; community care referrals; care planning in accordance with regulatory requirements; collaboration with the continuum care team; ongoing monitoring of plan effectiveness and reassessment of the need for Care Management (CM) services.

Required Qualifications:
  • Nursing degree/diploma required upon hire.
  • Bachelor's degree in Nursing or higher within 3 years of hire.
  • Upon hire: Oregon Registered Nurse License
  • 2 years Acute care hospital or community based experience.
  • 1 year Experience with electronic medical records or documentation systems.
  • 1 year Home health, mental health, substance use, hospice, utilization review, discharge planning or care management experience.


Preferred Qualifications:
  • Bachelor's degree in nursing upon hire.
  • Upon hire: Certified Case Manager
