Full Job Description
Crouse's Care Coordination Services team is hiring a Care Manager to review the services delivered to patients and compare to evidence-based guidelines in an effort to ensure medical necessity and appropriate level of care. This position will operate within the Crouse Health Neonatal Infant Care Unit (NICU).

Pay Range: $80,000.00 - $87,720.00 / year, based on education & experience

Care Managers are eligible for up to a $10,000 Sign-On Bonus!

Available Schedule(s): FT, Monday-Friday, Day Shift

Care Manager Position Responsibilities:
  • Works collaboratively with the multidisciplinary healthcare team to ensure the appropriate allocation of hospital resources while maintaining the quality of care.
  • Participates in the utilization management process using standards of care to determine the most appropriate level of care, managing care across the continuum to ensure a safe discharge in a timely manner.
  • Conducts the initial assessment of the patient's discharge plan based on the clinical course and proactively interacts with the healthcare team to establish the plan of care.

Care Manager Position Requirements:
  • Must have a current New York State Registered Nurse (RN) license.
  • Graduate from an accredited nursing degree program required.
  • Bachelor of Science degree in Nursing preferred.
    • For RNs licensed in or after 2017, completion of a Bachelor's of Nursing Degree will be required within 8 years of the date the license was obtained.
  • Certification in a specialty area preferred.
  • Three (3) - Five (5) years acute hospital experience.
  • Case Management or Utilization Management experience preferred.
