Cardiology Nurse Practitioner or Physician Assistant

Palm Coast, Florida | Full-Time

First Coast Heart & Vascular is hiring a full-time Nurse Practitioner or Physician Assistant to join its collaborative cardiology team in Palm Coast, Florida.

This position is primarily based in the outpatient clinic, with limited hospital coverage. The incoming provider will work closely with specialists in electrophysiology, interventional cardiology, and general cardiology while caring for patients with a variety of cardiovascular conditions.

Experienced APPs and new graduates are welcome to apply. Providers who are new to cardiology will receive support and mentorship from an established team of physicians and advanced practice providers.
Position Responsibilities
  • Perform new patient evaluations and established patient follow-up visits
  • Assess and manage patients with acute and chronic cardiovascular conditions
  • Develop individualized treatment plans in collaboration with cardiologists
  • Order and review EKGs, laboratory studies, cardiac imaging, stress tests, and other diagnostic testing
  • Manage cardiovascular medications and monitor patient progress
  • Educate patients and families about diagnoses, treatments, medications, lifestyle changes, and risk reduction
  • Coordinate care with cardiologists, nurses, referring providers, and other members of the healthcare team
  • Complete accurate and timely documentation in the electronic medical record
  • Assist with shared hospital rounding and call coverage
Schedule and Coverage
  • Full-time, Monday through Friday clinic schedule
  • Primarily outpatient responsibilities with limited hospital coverage
  • Shared weekend hospital call rotation with another APP
  • Remote weekday evening call coverage from 6:00 p.m. to 8:00 a.m.
  • Monday off following a weekend of call coverage
