The Advanced Practice Registered Nurse (APRN) - Value-Based Care Education & Clinical Onboarding serves as a clinical educator and subject matter expert responsible for developing, coordinating, and delivering education related to value-based care (VBC), Accountable Care Organizations (ACOs), population health, and quality initiatives. This role partners with clinical leadership, providers, care management teams, and operational departments to ensure new and existing clinicians are equipped with the knowledge and tools necessary to succeed in a value-based healthcare environment.

The APRN promotes evidence-based practice, quality improvement, patient-centered care, and regulatory compliance while supporting organizational goals related to clinical outcomes, patient satisfaction, and financial performance.

Position Summary

This role partners with clinical leadership, providers, care management teams, and operational departments to ensure new and existing clinicians are equipped with the knowledge and tools necessary to succeed in a value-based healthcare environment.

Essential Responsibilities

Provider Onboarding & Education

Design, coordinate, and facilitate onboarding programs for physicians, advanced practice providers, and clinical staff.
Educate providers on value-based care principles, ACO participation, quality measures, risk adjustment, documentation, and population health strategies.
Develop orientation materials, educational resources, competency assessments, and ongoing learning opportunities.
Serve as a clinical mentor and resource for newly onboarded providers.

Value-Based Care & ACO Support

Provide education on value-based reimbursement models, including shared savings programs, quality incentive programs, and alternative payment models.
Assist providers in understanding quality metrics such as HEDIS, CMS Star Ratings, MIPS, and other payer-specific performance measures.
Collaborate with leadership to identify educational opportunities that improve quality outcomes and reduce unnecessary healthcare utilization.
Promote best practices in preventive care, chronic disease management, care coordination, and transitions of care.

Clinical Quality Improvement

Analyze quality and performance data to identify educational needs.
Participate in quality improvement initiatives designed to improve patient outcomes and organizational performance.
Support implementation of evidence-based clinical guidelines and standardized workflows.
Assist with clinical documentation improvement initiatives to ensure accurate coding and risk adjustment.

Collaboration

Partner with medical directors, clinical leaders, care managers, quality teams, and operational leadership.
Participate in interdisciplinary committees related to quality, education, and value-based care.
Serve as a liaison between providers and organizational leadership regarding clinical education initiatives.

Regulatory Compliance

Maintain current knowledge of CMS regulations, ACO requirements, value-based payment programs, and healthcare quality standards.
Ensure educational programs align with regulatory requirements and organizational policies.

Qualifications

Education

Master's or Doctorate degree in Nursing from an accredited program.

Licensure

Current unrestricted APRN license.
National board certification appropriate to specialty.
Current DEA registration (if applicable).

Experience

Minimum of three years of clinical APRN experience.
Experience in value-based care, population health, managed care, or ACO operations preferred.
Experience developing and delivering provider education strongly preferred.
Quality improvement or healthcare leadership experience preferred.

Knowledge, Skills & Abilities

Strong understanding of value-based care models and accountable care organizations.
Knowledge of quality metrics, including HEDIS, CMS Star Ratings, MIPS, and risk adjustment (HCC coding).
Excellent presentation, teaching, coaching, and facilitation skills.
Strong analytical and problem-solving abilities.
Ability to interpret quality data and translate findings into educational initiatives.
Excellent communication and interpersonal skills.
Experience with electronic health records and clinical documentation optimization.
Ability to lead change and influence interdisciplinary teams.

Preferred Certifications

Lean Six Sigma (Green Belt or higher)
Certified Professional in Healthcare Quality (CPHQ)
Population Health or Care Management certification (preferred)

Compensation Range:
$124,681.00
to
$187,022.00

The anticipated base salary range represents the Company's good-faith estimate of the compensation it reasonably expects to pay for this position at the time of posting. Actual compensation will be determined based on factors including experience, skills, qualifications, geographic location, internal equity, and business needs.
