Position Summary

Associate Manager of Community Health Worker's

Location: Work at Home (anywhere USA)

Schedule: Mon-Fri 08:00-05:00MUST WORK EST HOURS)

The Associate Manager will lead the implementation, oversight, and ongoing management of the Community Health Worker (CHW) Pilot focused on addressing health disparities and improving outcomes among Medicaid members with Type 2 diabetes or other chronic diagnoses.

This role is critical in responding to identified program needs, including high disease prevalence, elevated social determinants of health (SDoH) risk, and disproportionate concentration of high-risk members in targeted counties. The Associate Manager will ensure the CHW program effectively improves member engagement, reduces avoidable utilization (ED visits and hospitalizations), and increases connection to primary care and community-based resources.

The Associate Manager will oversee CHW staff, ensure alignment with interdisciplinary care teams, and drive achievement of pilot goals, including improved self-management, enhanced member experience, and measurable improvements in quality and utilization metrics.

Position Responsibilities
  • Oversee implementation and day-to-day operations of the CHW pilot, ensuring alignment with program design, goals, and timelines.
  • Supervise CHWs, including caseload assignment (up to ~30 members per CHW), geographic alignment, and performance oversight.
  • Ensure timely member outreach, engagement, and completion of required assessments (e.g., screening within 30 days, outreach attempts).
  • Drive CHW activities focused on closing gaps in care, improving diabetes management, and increasing primary care utilization.
  • Ensure CHWs effectively connect members to SDoH resources (housing, food, transportation) and manage closed-loop referrals.
  • Partner with interdisciplinary care teams (care management, behavioral health, pharmacy, providers) to coordinate member care plans.
  • Lead case reviews, interdisciplinary rounds, and performance discussions to ensure member needs are addressed.
  • Monitor pilot performance metrics including ED utilization, inpatient admissions, primary care utilization, and HEDIS/CMS quality measures.
  • Identify risks, barriers to engagement, and operational gaps; implement corrective action plans.
  • Oversee documentation quality and compliance within care management systems (e.g., Dynamo).
  • Support pilot evaluation activities, including data reporting, analysis, and collaboration with internal and external stakeholders.
  • Promote CHW workforce development through coaching, training, and retention strategies.
  • Ensure adherence to Medicaid, contractual (DMAHS), and regulatory requirements.


Required Qualifications
  • 3-5 years of experience in care management, population health, community health, or Medicaid program operations.
  • Prior supervisory or team leadership experience.
  • Demonstrated experience managing programs targeting high-risk or underserved populations.
  • Strong understanding of SDoH, care coordination, and community-based interventions.
  • Experience managing performance metrics and driving quality and utilization outcomes.
  • Strong communication, collaboration, and analytical skills


License Requirements

  • Registered Nurse in state of residence
  • or
  • Independently licensed social worker/therapist in state of residence.


Preferred Qualifications
  • Experience leading or supporting CHW or community-based pilot programs.
  • Familiarity with NJ Medicaid program requirements, DMAHS, and HEDIS/quality metrics.
  • Experience with diabetes management initiatives or chronic disease programs.
  • Experience with care management systems (e.g., Dynamo) and community resource platforms.
  • Background in program implementation, evaluation, or pilot design.
  • Multilingual skills reflecting community population needs.


Education Required
  • Bachelor's degree in Public Health, Social Work, Nursing, Health Administration, or related field (or equivalent combination of education and experience).

Pay Range

The typical pay range for this role is:

$88,374.00 - $190,344.00

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
