A Associate Director of CalAIM Services (ADCS) contributes to AHF by blending the Mission and Core Values with care coordination processes and functions. The ADCS provides operational oversight of Enhanced Care Management (ECM) and CalAIM services across populations of focus, guiding the program evolution from implementation to optimization and sustainability. The ADCS maintains accountability for ECM performance metrics including enrollment, engagement, retention, service delivery timeliness, and outcomes, manages the CalAIM Community Health Worker & Basic Population Health Management Programs, monitors and analyzes Community Supports utilization, translates DHCS policy, All Plan Letters (APLs), and MCP contract requirements into operational strategy and is responsible for the establishment and achievement of the Care Management Division (CMD) goals and objectives for all CalAIM programs and services. The ADCS collaborates with Care Management, Clinical Operations, Quality, Finance, IT, and Community-Based Organizations to ensure seamless coordination between ECM, Community Supports, utilization management, and care coordination; thereby, improving health outcomes, promoting health equity, overcoming Social Determinants of Health (SDOH) barriers, and improving patient satisfaction and retention rates. The ADCS demonstrates strong leadership skills, clinical skills, proactive thinking, comprehensive knowledge of healthcare rules and supportive services, with a special focus dedicated to HIV/AIDS members, ensuring that all members have access to a comprehensive set of services based on their needs and preferences across the continuum of care.

The ADCS will work as a part of a close-knit team to maintain a comprehensive knowledge of the DHCS CalAim regulatory requirements, the CMD Model of Care, Health Plan Accreditation Standards, and county/state regulations and guidance, including All Plan Letters and contract deliverables, and provide updates to the team on a routine basis. The ADCS monitors ECM-related financial performance, staffing models, and resource utilization, supports budgeting, forecasting, and contract negotiations related to CalAIM services, identifies risks and opportunities related to reimbursement, performance incentives, and sustainability, directly manage ECM staff, providings mentorship, and establishes standardized workflows, policies, and reporting structures.
Licenses and Certifications: CA RN License required; BSN preferred
