- A Registered Nursing Degree from an accredited college or university.
- Minimum of two (2) years of experience as a nursing supervisor in a Healthcare Facility, Nursing Home, or other Long-Term Care (LTC) or Skilled Nursing Facility (SNF).
- Minimum of two (2) years of clinical nursing experience in wound care or related specialties.
- Current unrestricted license as a Registered Nurse (RN) in practicing state of PA prior to appointment.
- Wound care certification (WCC, CWOCN, or CWON or equivalent) is required prior to appointment.
- Current CPR/BLS certification prior to appointment.
Position Summary:
Reporting to the Director of Nursing, the Assistant Director of Nursing (ADON) - Wound Nurse will specialize in assessing and treating patients with complex wounds, ostomies, or continence conditions. They will work closely with members of patient care teams to ensure that personalized treatment plans are followed correctly in order to prevent future infection or injury.
This position is also responsible for planning, organizing, developing and directing the overall operations of the Nursing Service Department in accordance with local, state and federal standards and regulations, established facility policies and procedures. May be directed by the Administrator and the Medical Director, to provide appropriate care.
There are essentially three different ADON tracks, general operations and a quality focus. A job incumbent is expected to be able to perform functions of all tracks at any given time, especially in the case of resource support needs. The main differentiator is the amount of time spent performing said functions, that is general operations, quality focus, or wound care.

Duties:
Clinical Care Oversight: - Ensures delivery of compassionate quality care and nursing supervision as evidenced by adequate staff coverage on the units, general cleanliness, and adequate staff coverage on the units, creating a homelike environment, and adherence to resident's plan of care.
- Collaborates with members of the interdisciplinary team, physicians, consultants, and community agencies to identify and resolve issues and improve the quality of services.
- Performs rounds to observe residents and ensure nursing needs are being met.
- Conducts observations of nursing care and supervises development of in-service education to ensure nursing staff is competent in current knowledge and skills.
- Assists in oversight of resident incidents and concerns daily to identify any unusual occurrences and reports them promptly to the Director of Nursing, Administrator, and/or state agency for appropriate action.
- Monitors for allegations of potential abuse or neglect, or misappropriation of resident property and participates in the investigative process.
Wound Care Oversight- Conducts comprehensive assessments of patients' wounds and overall health status to develop individualized care plans.
- Implements preventative measures for at-risk patients, including pressure ulcer prevention and skin integrity maintenance
- Applies appropriate wound dressings and treatment modalities based on the specific needs of each patient.
- Educates patients and their families about wound care management, prevention strategies, and ongoing treatment plans.
- Collaborates with interdisciplinary teams, including physicians and physical therapists, to ensure cohesive patient care.
- Performs rounds with partnered wound care providers.
- Supervises and train nursing staff and other healthcare team members on proper wound care techniques and best practices.
- Maintains accurate and detailed documentation of wound assessments, treatment plans, and patient progress.
- Manages inventory of wound care supplies, ensuring adequate stock levels and timely reordering of materials.
- Coordinates schedules for patient follow-up visits and wound care consultations.
- Monitors and evaluates patients' responses to treatment, adjusting care plans as necessary to optimize healing.
- Stays updated on the latest advancements in wound care technologies and treatment options to provide the highest quality of care.
Operations and Management:- Supports and assists the Director of Nursing in planning, developing, organizing, implementing, evaluating and directing the day-to-day operations of the Nursing Services department, as well as its programs and activities, in accordance with current state and federal laws and regulations.
- Interprets and communicates policies and procedures to nursing staff; Monitors staff practices and implementation.
- Assists staff scheduler with oversight of nursing schedules to ensure resident needs, regulatory and budget standards are met.
Quality Assurance Oversight: - Monitors and evaluates the quality and efficiency of healthcare delivery processes to identify areas of improvement
- Develops, implements, and maintains adherence to quality assurance programs, policies, and procedures.
- Analyzes data and compile reports on quality indicators, trends, and performance metrics to identify potential areas for enhancement.
- Leads performance improvement projects and initiatives to enhance resident care outcomes.
- Keeps current on industry trends, research, and advancements in quality assurance practices to promote continuous improvement.
Risk, Compliance, and Safety Management:- Participates in QAPI and mock surveys; Implements correction plans.
- Reports noncompliance to the appropriate parties or stakeholders. May handle or coordinate resolution. (e.g. noncompliance, HR issues, patient/resident concerns, occupational hazards, safety, etc.).
- Complies with standards of business conduct, state/federal regulations, and guidelines.
- Establishes a culture of compliance by adhering to all facility policies and procedures.
Human Resources Management:- Guides, directs, and manages subordinate supervisor staff, and their teams as necessary.
- Assists in evaluating work performance of all nursing personnel and implements corrective action according to operational policies.
- Participates in the recruitment, hiring and termination of nursing personnel as indicated.
Knowledge, Skills, and Abilities
Knowledge of:
- Current state and federal regulations, practice standards, guidelines, policies, procedures etc. including, but not limited to, the following
- Center for Medicare and Medicaid Services (CMS) regulations.
- Long Term Care (LTC) and Skilled Nursing Facility (SNF) regulations and guidelines.
Skill in:
- Microsoft Office Suite
- Nursing and compassionate patient care and advocacy.

- Plan, organize, direct, control, coordinate, and evaluate the quality and appropriateness of care and treatment provided by both professional nursing staff and direct care staff in all resident areas.
- Ensure delivery of compassionate quality care and nursing supervision as evidenced by adequate staff coverage on the units, general cleanliness, and maintaining optimal resident functions.
- Promote safe work practices, safety rules, and accident prevention procedures to prevent employee injury and illness.
- Meet general health requirements according to facility policy, including medical and physical exams and checking immunity status to various infectious diseases.
- Assist in evacuating residents during emergency situations.
- Work beyond normal working hours and on weekends and holidays as necessary. On call 24 hours per day, 7 days per week.
- Sit, stand, and walk throughout shift or for long periods of time.
Residency: Allegheny County within 1 year.
