Job Title: Assistant Director of Clinical Services
Location: San Francisco - War Memorial Building (Office)
Travel across all Swords to Plowshares locations
Schedule: Monday through Friday, 7am to 3pm or 8am to 4pm
Reporting Type: In person, option for 1 day remote / hybrid after the first 90 days
Employment Type: Exempt, Full-time
Salary Range: $140,000 annual
Summary:
This position reports to the Director of Clinical Services.
This position serves as the organization's lead for clinical documentation quality and audit readiness. This position oversees the quality and consistency of clinical documentation across all Swords programs and works to ensure the organization is always prepared for monitoring and audits. Working with leadership, IT, Finance, and Grants, this position will develop documentation standards, reviews charts, trains staff and identify opportunities to improve documentation and compliance.
Roles and Responsibilities
Clinical Quality & Documentation
• Establish, implement, and maintain organization-wide standards for clinical documentation, assessments, treatment planning, and case record quality.
• Oversee client files, charting, and other documentation through regular and ongoing internal audits and reviews.
• Review and approve clinical documentation within EPIC and other electronic health record systems, as required.
• Analyze trends and systemic improvement opportunities and partner with to document issues, implement corrective plans, and develop training to lead quality improvement.
• Work with leadership and IT to digitize files in accordance with industry standards.
Compliance and Audit Readiness
• Stay current on regulatory, grant, and contract requirements related to clinical documentation and serve as the organization's subject matter expert.
• Attend funding, contract, and compliance meetings to monitor changes in requirements and ensure consistent implementation across programs.
• Develop standard operating procedures related to clinical documentation and compliance and train staff in their implementation.
• Ensure adherence to laws regarding confidentiality, privacy, documentation, and reporting requirements.
Internal and External Reporting
• Attend audits to keep notes on findings and draft a written response to any audit findings for leadership review.
• Implement corrective action plans resulting from audit findings in a timely manner.
• Keep leadership informed of emerging compliance risks, grant performance requirements, documentation trends, and opportunities for quality improvement.
Leadership Support and Operational Coverage
• Partner with the Director to address emerging operational, staffing, and clinical issues and ensure continuity of services across programs.
• In rare circumstances, respond to after-hours phone calls from staff to answer clinical questions, assist with coordinating coverage for unexpected staff absences, or be onsite in emergencies when other designated team members are unavailable.
• Represent the Director at meetings, committees, and other internal or external functions as assigned.
• Assume additional responsibilities or special projects as assigned.
Requirements
Education
• Master's degree in counseling, social work, or related field
• Valid California BBS License required (MFT, LCSW, LPCC) in addition to the ability to supervise BBS Interns.
• Training in the use of DSM V - TR to develop provisional diagnosis
• Training and familiarity with treatment protocols for people with mental and behavioral health diagnoses.
• Must maintain a valid Authorization to Write 5150/5585 Applications card in San Francisco City & County.
Experience
• Operating or supervising a Medi-Cal compliant clinical outpatient program.
• Understanding of veterans and homelessness and challenges with readjustment.
• Multi-disciplinary treatment planning, substance abuse counseling, and crisis management.
Qualifications
• Must be able to communicate effectively with a variety of personalities.
• Commitment to treating all clients and colleagues with dignity and respect.
• Knowledge of San Francisco Department of Public Health programs or Public Health resources is preferred.
• Technological proficiency: Microsoft Office, EPIC and client management software.
• Interpersonal and supervisory: conflict resolution, crisis intervention, quality assurance, staff development, and performance appraisal.
• Fluency in English and ability to effectively read, write, and communicate in English with staff, clients, and external partners and stakeholders.
