Performs complex activity in the performance, administration and supervision of diagnostic, habilitative and rehabilitative services to patients with speech and language impairments in order to assist them towards the attainment of greater effectiveness in the process of communication by addressing communicative disorders indicated by impaired voice, language or speech, and ameliorating the emotional and psychological adjustment to such disorders; performs related work.
Example of typical tasks:
With wider latitude for independent judgment, including supervisory responsibilities, performs duties and responsibilities described under Assignment Levels I and II:
1. Provides assistance to departmental leadership in a wide-range of activities in the management of a health care setting and/or medical center, including such areas as health care setting administration and administrative systems, professional, operational and related support activity services.
2. Assists in the administration management of specified activities and functions. Carries out assignments and undertakes studies involving the total service center or specific patient services, including budget planning, cost studies/analysis, and preparing specific budgets.
3. Collects and analyzes data needed as a basis for administrative and management decisions relative to various health care setting activities.
4. Accumulates data for budget preparations, fiscal reports, and review of budget estimates.
5. Investigates and reports on patient and visitors complaints and performs special studies and investigations on operational units.
6. Prepares and develops informational data in health care setting training programs, and participates in the development and updating of activity programs.
7. Plans, directs, coordinates, controls and reviews work of subordinate administrative personnel relating to assigned tasks and projects in health care setting administration, health care setting management and related fields.
8. Implements new programs and projects and monitors their operation and effectiveness.
9. Performs other related duties, as directed.
In additional to performing the duties and responsibilities described under Assignment Levels I and II, performs the following:
1. Supervises and coordinates functional activities of the health care setting with other organizational units and maintains liaison with internal and external partners; prepares reports related to functional activities.
2. Assists in the preparation of operating plans, policies, programs and procedures; assists or collaborates with appropriate staff in resolving operating problems to facilitate solutions.
3. Coordinates and plans training programs and activities to establish performance standards; provides education and training to staff, as needed.
4. May represent departmental leadership on various internal and external committees and in relevant functions and programs.
Minimum Qualifications1. Master's degree from an accredited college or university with a specialization in hospital administration,
health care administration, public health or a related discipline; or
2. Baccalaureate degree from an accredited college or university in disciplines, as described in "1" above,
and one (1) year of administrative experience; or
3. An equivalent combination of training, college education and experience in related fields.
Department Preferences1. Provide daily oversight of an assigned panel of high-need candidates/members, delivering interventions designed to reduce unnecessary emergency department (ED)/emergency room (ER) and inpatient (IP) utilization, increase connection to primary care/behavioral health care, and demonstrate improved health outcomes for the served population. Communicate changes in member's status (e.g. mental health, medical, social determinants impacting health and wellness) to the care team.
2. Administer program requirements as defined by all applicable federal, state, local, and agency guidelines (e.g. assessments, care plans, documentation of services, etc.) and other Health Home documentation required to address member needs.
3. Monitor alerts (e.g. hospital admission/discharge, incarceration, ER/ED visit, etc.) and provide onsite follow up with members promptly, verifying appropriate levels of care and access to all needed medications, therapies, and supports in managing care transitions.
Provide regular, field based face-to-face visits with members/candidates and providers in home and /or community settings, ensuring core services and HML are completed monthly.
5. Participate in case conferences as needed to ensure that each member's care team is aligned with a shared plan of care and up-to-date information on each member. Provide written status updates on a regular basis as requested (case conference tool, emails, etc.) and verify documentation of all such activities in member chart.
6. Adhere to timeframes and quality standards regarding completion of member assessments, reassessments, care plans, progress notes and documentation standards (including HARP Eligibility Assessment, HARP Plan of Care, LOSD, case notes, updates, consents, etc.).
7. Manage schedule to allow for adequate field time, travel time, and time to complete administrative duties of position. Maintain communication with supervisor on changes in schedule and maintain an updated Outlook calendar of appointments.
8. Maintain positive partnerships with providers and clinics internal to (and external to as necessary) NYC Health + Hospitals system.
9. Maintain a directory of provider services including medical, behavioral health, social support and other services as needed to support referral, linkage and coordination activities of assigned members. Identify valuable resources to share with the rest of the team to the benefit of all members.
10. . Support staff onboarding and growth as a mentor/coach.
11. Minimum of 2 years' experience in care coordination, care management, case work, with chronically ill, HIV/AIDS, homelessness, and substance abuse patients
