Job Summary

Sarasota Memorial First Physicians Group has an exceptional opportunity for an experienced Family Nurse Practitioner or Physician Assistant in our Sarasota market.

The ideal candidate will bring a combination of clinical experience, strong communication skills, familiarity with telehealth and transitional care, and an interest in working within a new team and program. Flexibility, initiative, collaboration, and the ability to develop effective workflows in a newly established role will be important to success.

Opportunity Highlights:
  • Monday - Friday, 8AM - 5PM
  • Work within two clinical teams in a non-clinical setting overseeing remote nurse triage services and Transitional Care Management of First Physicians Group primary care patients.
  • No evenings, no weekends, no holidays, no on-call responsibilities.
  • Reviews and oversees patients transitioning from hospitals, skilled nursing facilities, rehabilitation facilities, and other qualifying settings.
  • Determines whether the patient's condition is appropriate for telehealth or requires in-person evaluation, emergency care, laboratory testing, imaging, or another level of care.
  • Performs an appropriate and accurate history and virtual physical assessment using available technology and patient-assisted examination techniques when clinically appropriate.
  • Reviews discharge summaries, medication changes, diagnostic results, treatment plans, and outstanding clinical
  • Provides or oversees the required clinical follow-up and medical decision-making associated with TCM.
  • Coordinates care with primary care providers, specialists, home health, facilities, caregivers, pharmacies, and other members of the healthcare team.
  • Reviews TCM documentation for clinical completeness, accuracy, and compliance.
  • Educates and provides clinical guidance to nurses, care coordinators, medical assistants, and other staff involved in the TCM process.

Requirements:
  • Graduate from accredited APRN or PA program

The Advanced Practice Registered Nurse (APRN) for Transition Care Management (TCM) & Nurse Triage reflects the mission, vision, and values of Sarasota Memorial Health Care System, adheres to the organization's Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines, and other regulatory and accreditation standards. An APRN is a registered nurse and therefore can perform all duties that are considered routine duties of a registered nurse. In addition, an APRN holds a master's or doctoral degree. The APRN functions as part of the multidisciplinary health team in accordance with privileges approved by the credentialing committee to provide high-quality, cost-effective advanced nursing care to patients within the APRN's scope of practice.

Required Qualifications

- Require advanced degree from an accredited school or college of nursing* *Must have had a Medicare number in the State of Florida prior to 1/1/03; if not, must have a master's degree in nursing in addition to being a Registered Professional Nurse who is authorized by Florida to practice in accordance with state law.
- Require approved APRN board certification**. **Must be certified as a APRN by a recognized national certifying body having established standards for APRN's. The following organizations are recognized national certifying bodies for the Nurse Practitioner: American Academy of Nurse Practitioners; American Nurses Credentialing Center; National Certification Corporation for Obstetric, Gynecologic and Neonatal Nursing Specialties; National Certification Board of Pediatric Nurse Practitioners & Nurses; Oncology Nurses Certification Corporation; Critical Care Certification Corporation; National Commission on Certification; American Association of Heart Failure Nurses; and the American Midwifery Certification Board (AMCB).

- Prefer effective interaction skills to ensure the successful care of patients in the clinical setting.
- Prefer effective leadership and strong interpersonal skills.
- Prefer demonstrated ability to work independently and effectively with physicians and other allied health personnel in the hospital and/or office setting.
- Prefer demonstrated skills in documentation in medical records to substantiate accurate billing.
- Prefer knowledge of relevant state and federal laws, statutes and regulations.
