AdventHealth Well 65+ is seeking an experienced
APRN or Physician Assistant to provide personalized, high-quality care to patients 65 years and older in a value-based, Medicare Advantage practice focused on quality, outcomes, and meaningful patient relationships.
We have a growing network of community-based practices. Our providers benefit from a well-established, integrated health system that ensures seamless access to hospitals, specialists, and coordinated care resources, allowing patients to thrive through comprehensive care.
- Manageable patient panel: Maximum 200 patients
- Low daily volume: 10-14 patients per day
- Minimal call requirements
- Robust interdisciplinary care team including RN case management, health coaches (MAs), pharmacy, social work, care coordination, HCC coding support, and dedicated practice leadership

Education & Experience - Master's degree in nursing (APRN/FNP) or Physician Assistant Studies
- Must have 2+ years of clinical experience as an APRN or PA-C
Licensure, Certification & Registration - Active Florida APRN or PA-C license
- Current DEA license
- ANCC or AANP Certification
- BLS required; ACLS (preferred)
