Description

The Advanced Practice Registered Nurse serves Lawrence-Douglas County Public Health by performing Advanced Practice Registered Nurse duties in clinical assessments and client needs. Assisting in the management of health and wellness for Lawrence-Douglas County Public Health clients.

Essential Job Functions:

Provide direct services to clients to include:
  • Provide preventive care, health screenings, physical exams, community-based primary care, and immunizations per Lawrence-Douglas County Public Health protocols
  • Provide assessment, diagnosis, and treatment for common acute and chronic conditions
  • Conduct patient histories, physical exams, diagnostic testing, and treatment planning
  • Provide reproductive health services, including contraception counseling, STI testing and treatment, pregnancy testing, and basic gynecologic care
  • Offer education and counseling on family planning options, contraceptive methods, and preconception health to support patients' reproductive choices and overall health goals
  • Develop and implement patient care plans based on evidence-based practices
  • Perform laboratory tests and measurements and provide results to clients
  • Provide low-level psychiatric medication management, including assessment, prescription, and follow-up for conditions such as mild depression, anxiety, and other common mental health concerns within scope of practice
  • Initiate, adjust, and monitor low-level psychotropic medications, in collaboration with behavioral health specialists.
  • Provide accurate, evidence-based education and counseling to clients
  • Educate patients on wellness, chronic disease prevention, lifestyle choices, safe sex practices, medication adherence, and other public health issues to reduce health risks and improve community well-being
  • Promote health and disease prevention through services such as immunizations, blood pressure monitoring, diabetes screenings, and other chronic disease screening
  • Provide resources and guidance to patients and families for managing both physical and mental health conditions.
  • Promote a holistic approach to health, incorporating physical and mental well-being in treatment plans.
  • Maintain accurate and timely documentation in electronic health records.
  • Ensure compliance with all local, state, and federal regulations related to patient care, safety, and medication management
  • Refer patients as appropriate and serve as client advocate
  • Consult with medical consultant as appropriate

Plan, implement, evaluate, and improve organizational programs in area of responsibility
  • Apply QI principles to meet or exceed performance standards
  • Identify, collect and analyze data to assist with program planning, evaluation and improvement
  • Exhibit critical thinking skills to effectively lead area of responsibility

Coordinate program activities in coordination with Director of Clinic Services to include:
  • Development of program objectives and provide quality guidance
  • Monitor grant requirements and provide timely communication about program changes
    • Assist with completion and submission of grant reports as required
  • Develop, implement, train and monitor policies, procedures and forms to ensure quality delivery of services in conjunction with the Director of Clinic Services
    • Interpret and enforce policies and procedures
    • Analyze and evaluate program activity to ensure efficiency and effectiveness
    • Monitor external changes in the technical and health system that impacts the programs
  • Represent the Department to the community and other key stakeholders
  • Build and maintain collaborative relationships with key stakeholders
  • Provide health education presentations for community awareness
  • Participate in community outreach activities, health fairs, and public health initiatives to engage underserved populations and raise awareness about public health services

Requirements

Requirements include a Master's degree in nursing, hold and maintain an APRN license to practice in Kansas, current DEA License. Knowledge of the nursing process, principles, and procedures in public health nursing is also required.
