About the Position

As a Family Nurse Practitioner (APRN) with Healthy Connections, you will deliver integrated, high-quality care to patients of all ages. This role is ideal for a provider who values teamwork, evidence-based medicine, and a holistic approach to health - including support for patients receiving Medication Assisted Treatment (MAT).

You will work closely with a collaborative team of physicians, behavioral health professionals, and support staff to ensure patients receive comprehensive, judgment-free care.

In this particular role, we are looking for an APRN with experience, willing to see 25-30 patients per day.

Key Responsibilities
  • Perform physical exams, patient assessments, and detailed medical, family, and social histories.
  • Diagnose and manage acute and chronic illnesses in a primary care setting.
  • Provide patient-centered Medication Assisted Treatment (MAT) services and support adherence to treatment plans.
  • Order, interpret, and follow up on diagnostic and laboratory tests.
  • Prescribe medications and treatments as appropriate.
  • Educate and counsel patients and families on health conditions, treatment options, and lifestyle changes.
  • Coordinate care with supervising physicians, specialists, and other team members.
  • Participate in quality improvement initiatives, including Patient-Centered Medical Home (PCMH) and Meaningful Use (MU) goals.

Qualifications
