POSITION SUMMARY:

The Advanced Practice Provider (APP), either a Physician Assistant or Nurse Practitioner, will serve as the primary front-line clinician for the Single Ventricle Home Monitoring Program (SV-HMP) and support the Single Ventricle Outcomes Network (SV-ONE) and Cardiac High Acuity Monitoring Program (CHAMP) registries. Working under physician supervision and in collaboration with the multidisciplinary congenital heart team, the APP will provide coordinated clinical care for single ventricle/high risk inter-stage patients, support family education and longitudinal engagement, and help maintain accurate, compliant registry and quality-improvement operations.

This role reports to the SV-HMP Director/SV-ONE Site Principal Investigator, with overall oversight from the Division Chief of Pediatric Cardiology. The incumbent collaborates closely with inpatient congenital cardiology, cardiac surgery, outpatient cardiology, nursing, nutrition, psychosocial services, hospital quality teams, and research administration. Responsibilities include providing clinical care (60%), supporting research and registry management (25%), leading quality improvement and educational initiatives (8%), and managing administrative and regulatory activities (7%).

Key Accountabilities
• Triage and evaluate abnormal home-monitoring readings, symptoms, and acute calls daily; coordinate timely escalation with the attending cardiologist.
• Lead weekly multidisciplinary rounds for SV-HMP patients and participate in weekly inpatient rounds for interstage single ventricle patients.
• Conduct weekly telehealth visits for enrolled SV-HMP patients, including interval history, focused assessment, review of monitoring trends, care-plan reinforcement, documentation, and follow-up coordination. In person visits as needed.
• Educate patients' families about SV-HMP goals, equipment, monitoring parameters, red-flag symptoms, communication expectations, and transition from hospital to home before discharge.
• Obtain informed consent for participation in SV-HMP, CHAMP, and SV-ONE in the inpatient or outpatient setting, as applicable.
• Coordinate hospitalizations, preoperative testing, surgical timing, and urgent clinic evaluation in collaboration with the responsible physicians and clinical teams.
• Facilitate multidisciplinary follow-up, including nutrition, hepatology, neurodevelopmental, psychology, and other specialty services as clinically appropriate.
• Provide comprehensive documentation, enter and follow clinical orders within scope of practice, communicate results and medication changes as directed, and make appropriate referrals.
• Serve as a clinical resource for families, primary care clinicians, and internal and external health care professionals caring for SV-HMP patients.
Registry Research and Quality Improvement
• Serve as the primary site clinician for SV-ONE and CHAMP, supporting complete enrollment, accurate longitudinal data, and timely resolution of data queries or audit discrepancies.
• Coordinate with designated hospital quality staff responsible for data entry and verify that required SV-ONE and CHAMP data are complete and current.
• Lead chart audits to assess adherence to single ventricle surveillance practices, including cardiopulmonary exercise testing, liver assessment, cardiac imaging, catheterization, and vaccination review, and notify primary providers when items are overdue.
• Collaborate with the SV-ONE Data Coordinating Center and UTHealth research offices on regulatory submissions, continuing reviews, audits, and data-use agreements.
• Support SV-ONE and CHAMP quality-improvement initiatives, including Plan-Do-Study-Act cycles, implementation of clinical pathways, monitoring of center performance measures, and documentation of pathway deviations.
• Coordinate patient-reported outcomes, quality-of-life surveys, and wearable-data initiatives as assigned.
• Participate in program-related clinical research, reports, abstracts, presentations, and publications as appropriate.
Programmatic Administrative and Educational Duties
• Serve as the primary clinical contact and representative for the SV-HMP team and maintain consistent communication with the program director, site principal investigator, and multidisciplinary team.
• Act as a liaison among the UTHealth SV-ONE team, the Cincinnati Children's coordinating center, hospital administration, research offices, and the UTHealth Fetal Center.
• Participate in national SV-ONE learning sessions, webinars, and relevant professional meetings; share emerging best practices with the local team.
• Provide ongoing education for fellows, nurses, APPs, and staff regarding interstage care pathways, SV-HMP responsibilities, and CHAMP data review.
• Review and integrate current literature, national learning-network guidance, and institutional protocols into clinical practice.
• Prepare patient and family letters, school or insurance documentation, and program materials as needed.

CERTIFICATIONS/SKILLS:

Nurse Practitioner: Current Texas Licensure as a Registered Nurse Current Texas Licensure as an APRN-NP, and specialty certification as required by department Basic Life Support (BLS) Certified
Physician Assistant: Current Texas Licensure as a Physician Assistant Current Physician Assistant certification issued by the National Commission of Certification of Physician Assistants Basic Life Support (BLS) Certified Proficient in the use of medical and surgical instruments and equipment required by work.

MINIMUM EDUCATION:

Nurse Practitioner: Masters in Nursing as NP or master degree in Nursing with completion of a post-master Nurse Practitioner Certification Program.
Physician Assistant: Master's degree as Physician Assistant. A Bachelor's degree as a Physician Assistant with at least two years of professional experience in the role is acceptable based on terminal degree requirements at the time of graduation.

MINIMUM EXPERIENCE:
• None, however, previous experience in pediatric cardiology or a related clinical area is preferred.
• Experience with clinical registries, research coordination, regulatory compliance, data-quality review is a helpful.
