This Advanced Practice Provider will join Community Health Network, a multispecialty group with over 1,300 providers in Indiana. With an appealing blend of big-city amenities, cosmopolitan style, and communities which boast top notch school systems, the Indianapolis area is consistently ranked nationally among the best places to live. Highlights
Community Health Network has an excellent opportunity for a board-certified, family medicine trained advanced practice provider to work in our Retail locations. Community Health Network's Community Clinics put high quality, convenient primary and acute care throughout all regions of Indianapolis and the surrounding suburbs.
  • Autonomous model of primary care in a fast paced environment with support of offsite collaborating physician; on-site pharmacist
  • Clinics are open Monday - Friday 8:30 A - 7:00 P; Saturday and Sunday 8:30 A - 5:00 P
  • Every other weekend commitment
  • Closed on Christmas, Thanksgiving and New Year's Day

Job Duties
Advanced Practice Provider will provide healthcare, including assessment, diagnosis, and treatment of acute and chronic health conditions and wellness preventative medicine. Promotes a healthy lifestyle and involves the client in his/her plan of care with emphasis placed on patient self-determination and maximization of client knowledge to enhance care of oneself.

Experience
Minimum two to five years of family medicine or urgent care experience as an advanced practice provider firmly required; this is an independent position requiring a strong level of autonomy

Education
Educational background consistent with requirements for licensure and certification with the State of Indiana

Community Health Systems is a Fortune 500 company based in Franklin, Tennessee. It was the largest provider of general hospital healthcare services in the United States in terms of number of acute care facilities.
